剖宫产分娩高额住院费用的新灰色关联分析

New grey correlation analysis of hospitalization costs of high cesarean section

目的分析了解某院近5年来剖宫产分娩高额住院费用的影响因素及发展态势，为合理控制高额剖宫产分娩，提高医疗质量、合理分配和有效控制单病种医疗费用提供量化的评价依据及有意义的参考。方法运用新灰色关联分析法，结合Excel汇总分析某院2008-2012年住院费用大于1万元的291例剖宫产分娩住院费用的9类构成进行定量分析。结果通过灰色关联度及关联序可知，治疗费是某院5年来剖宫产分娩高额住院费用最主要的影响因素，关联度为1．000；其次为药品费，关联度为0．890；第三位为手术费，关联度为0．854；接着依次为化验费、床位费、输血费、检查费、输氧费和放射费。结论经新灰色关联分析法分析，某院近5年剖宫产分娩高额住院费用影响因素所得关联序与实际情况相符。新灰色关联法能为系统发展变化的态势提供量化指标，方法可靠、合理，适用于因素间关联度的衡量及动态历程分析，在应用于高额住院费用影响因素分析上有较大的可行性和实际意义。

Objective To explore influencing factors and development tendency of high cesarean section hospitalization costs in recent 5years in a hospital, and to provide quantitative evaluation criterion and significant reference for reasonable control of high cesarean section hospitalization costs. Methods Quantitative analysis was conducted on composition of 9 cesarean section hospitalization costs of 291 patients with costs more than 10 thousand from 2008 to 2012 in the hospital with new grey relational analysis, combined with Excel pooled analysis. Results Grey correlation degree showed that treatment fees was the most important influencing factor in cesarean delivery high hospitalization costs in the past 5 years, with the correlation degree 1. 000, medication fees ranked the second, with the correlation degree 0.890, the surgery fees ranked the third, with the correlation degree 0. 854, and laboratory fee, bed fee, transfusion fee, inspection fee and costs of radiation followed in a descending order. Conclusion The new grey correlation analysis can provide quantitative indicators for systematically varying development profile with reliable and reasonable methods, and it is suitable for dynamic analysis on correlation between factors.
