凯妮汀用于妊娠期阴道念珠菌病的疗效观察

Observation on clinical efficacy of Canesten for the pregnancy vaginal candidiasis

目的探讨凯妮汀用于治疗妊娠期阴道念珠菌病的疗效。方法回顾性分析广西壮族自治区百色市田阳县百育镇卫生院2009年9月～2012年9月320例妊娠期阴道念珠菌病患者,其中2009年9月～2011年1月的142例患者使用制霉菌素泡腾片治疗(制霉菌素组),2011年2月～2012年9月的178例患者采用凯妮汀治疗(凯妮汀组),再将178例凯妮汀组患者根据用药时长的意愿分为3个亚组,分别为:单次用药组51例、2次用药组(1片/次,3 d 1次,共2次)69例与4次用药组(1片/次,3 d 1次,共4次)58例。比较各组的治疗效果、不良反应与复发率。结果凯妮汀组的治愈率明显高于制霉菌素组,差异有高度统计学意义(P<0.01);总有效率与无效率两组相当,差异无统计学意义(P>0.05);制霉菌素组的复发率较凯妮汀组高,两组相比差异有统计学意义(P<0.05)。三亚组中4次用药组治愈率最高,2次用药组次之,与单次用药组相比差异均有统计学意义(P<0.05);单次用药组、2次用药组与4次用药组的总有效率分别为90.2%、97.1%、98.28%,2次用药组、4次用药组与单次用药组相比差异均有统计学意义(P<0.05);复发率以单次用药组最高,与2次用药组、4次用药组相比差异有统计学意义(P<0.05)。结论凯妮汀治疗妊娠期阴道念珠菌病疗效明显,复发率低,妊娠期用药较为安全。

Objective To investigate the efficacy of Canesten treatment during pregnancy vaginal candidiasis.Methods A retrospective analysis of Baiyu Health Center of Tianyang County in Baise City in Guangxi Zhuang Autonomous Region from September 2009 to September 2012 was made.Totally 320 cases of pregnancy vaginal candidiasis pa tients was studied.From September 2009 to January 2011,there were 142 patients using nystatin effervescent.From February 2011 to September 2012,there were 178 patients using Canesten.According to medical duration,these 178 cases who received Canesten treatment were divided into three subgroups,as 51 patients in single dose group,69 cas es in double dose medication group（1 pill each time,1 time in 3 days,2 times totally）,58 cases in four dose medica tion group（1 pill each time,1 time in 3 days,4 times totally）.The clinical effects,adverse reactions and the relapse rate in three groups were compared.Results The cure rate of Canesten group was significantly higher than the nystatin group,the difference was significant（P ＆lt; 0.01）;there were no obvious differences in totally efficiency and inefficiency between Canesten group and nystatin group（P ＆gt; 0.05）.The recurrence rate in nystatin group was higher than Canesten group（P ＆lt; 0.05）.In three subgroups,four dose medication group had the highest cure rate,while double dose medica tion group was lower,and single-dose group was the lowest.The totally efficiency in single dose group,double dose group and four dose group were 90.2%,97.1%,98.28% respectively.The totally efficiency in double dose group and four dose group were higher than single dose group（P ＆lt; 0.05）.The recurrence rate in single dose group was the highest compared to the other subgroups（P ＆lt; 0.05）.Conclusion The clinical efficacy of Canesten for gestational vaginal candidiasis is affirmed.It has the advantage of low recurrence rate and safety during pregnancy.
