摘要
目的探讨兔皮瓣移植术后水肿与血管再生之间的关系。方法采用30只新西兰大白兔背部轴型皮瓣制作皮瓣模型,分别于术后1,3,7,14,21,28d切取皮瓣周边缝合区的皮瓣组织进行血管内皮生长因子免疫组织化学和HE染色,显微镜下观察皮瓣肿胀程度和阳性血管计数。结果皮瓣肿胀于术后开始出现,3d达高峰,7d基本消退,血管增生于术后3d开始出现,于术后7~14d达到高峰,28d后恢复至术前水平;血管内皮生长因子表达于术后3d开始出现,7d达到高峰。结论皮瓣水肿主要来源于血管回流障碍,血管再生呈阶段性,血管内皮生长因子间接影响血管再生和皮瓣水肿。
Objective To explore the relationship between edema and angiogenesis after flap transplantation in New Zealand white rabbits. Methods The shaft type flaps were used to establish the models from the backs of 30 New Zealand white rabbits. The flaps obtained from peripheral suture zone by day 1, 3, 7, 14, 21 and 28 after operation were treated with vascular endothelial growth factor (VEGF) immunohistochemical method and HE staining. The swelling degree of the flaps and the positive blood vessel counts were observed under the microscope. Results The edema occurred immediately after operation, reached the peak by day 3, and basically subsided by day 7. The angiogenesis began to appear by day 3 after operation, reached the peak by day 7 to 14, and returned to the preoperative level 28 days later. The VEGF expression began to appear by day 3 after operation, and reached the peak by day 7. Conclusion The flap edema is mainly caused by vascular reflux disorders. The angiogenesis presents the character of stage. The VEGF expression influences indirectly the vascular regeneration and flap edema.
