手十二井穴放血为主治疗脑卒中后抑郁状态临床观察

Therapeutic Observation on Blood-letting at Twelve Hand Jing-well Points for Post-stroke Depression

目的观察手十二井穴放血治疗脑卒中后抑郁(post stroke depression,PSD)的临床疗效。方法将60例PSD患者随机分入治疗组、针刺组和西药组,每组20例。3组均给予口服帕罗西汀,针刺组同时给予针刺,治疗组同时给予针刺和手十二井穴放血,治疗前、后分别进行HAMD评分及BI评定。结果治疗4星期后,治疗组总有效率为100.0%,针刺组为80.0%,西药组为75.0%,治疗组总有效率优于针刺组和西药组(P<0.05,P<0.01);治疗后治疗组HAMD与BI评分改善均优于针刺组(P<0.05),治疗组HAMD评分改善优于西药组(P<0.01),BI评分改善亦优于西药组(P<0.05)。结论手十二井穴放血治疗脑卒中后抑郁状态的疗效满意。

Objective To observe the therapeutic effect of blood-letting at twelve Hand Jing-well points in treating post-stroke depression （PSD）. Method Sixty PSD patients were randomized into a treatment group, an acupuncture group, and a Western medication group, 20 each. The three groups all received Paroxetine. In addition, the acupuncture group was given acupuncture treatment, and the treatment group was given acupuncture plus blood-letting at twelve hand Jing-well points. They were evaluated by using Hamilton Rating Scale for Depression （HAMD） and Barthel Index （BI） before and after treatment. Result After 4-week treatment, the total effective rate was 100.0% in the treatment group, versus 80.0% in the acupuncture group and 75.0% in the Western medication group. The total effective rate of the treatment group was significantly higher than that of the acupuncture group and the Western medication group （P〈0.05,P〈0.01）; after treatment, the improvements of HAMD and BI in the treatment group were better than that in the acupuncture group （P〈0.05）; the improvement of HAMD in the treatment group was significantly better than that in the Western medication group （P〈0.01）, and the improvement of BI in the treatment group was also markedly better than that in the Western medication group （P〈0.05）. Conclusion Blood-letting at twelve hand Jing-well points can achieve a satisfactory therapeutic effect in treating PSD.
