摘要
目的:探讨绝经后T2DM患者慢性并发症与骨密度(BMD)、骨质疏松症(OP)患病率情况的关系。方法:回顾性分析168例绝经后T2DM患者,收集年龄、体重、身高、腰围、臀围、病程及绝经年限等临床资料,应用双能X线骨密度仪测定间腰椎及髋部等各部位BMD值。将全体受试者按照糖尿慢性并发症分组,进一步按照各年龄组平均DM病程分组。结果:①除DN组Ward's区、DR组T-hip部位BMD值显著低于NC组(P<0.05)外,各组间腰椎及髋部等各部位的BMD值、OP患病率均无显著性差异(P>0.05)。不同病程的患者进行比较,年龄、绝经年限、BMI、HbA1c、各部位BMD值和OP患病率均无统计学差异(P>0.05)。②多元逐步回归分析提示,绝经后T2DM患者BMD值的主要影响因素为年龄、BMI、HbA1c以及绝经年限(P<0.05),而糖尿病病程、并发症类型均非BMD的独立影响因素。结论:未发现绝经后T2DM患者BMD及OP患病率与糖尿病慢性并发症(DN、DR、DPN)有相关关系。
Objective: To investigate the changes in bone mineral density(BMD),and prevalence of osteoporosis in postmeno-pausal women with type 2 diabetes mellitus(T2DM) with complications.Methods: 168 patients of postmenopausal women with T2DM were involved in this retrospective research.The clinical information of the patients were collected including gender,age,menopausal years,weight,height,duration of the disease.Parameters of BMD were also attained.All subjects were divided into groups according to complications and duration of disease.Results:①Only the BMDs of Ward's region in DN group and T-hip region of DR group were significantly lower than that in NC group.Other BMD of lumbar spine and hip were not significantly different among each group(P0.05).OP Prevalence had no significant difference between patients with various complications(P0.05).There were no statistical difference in age,menopause years,BMI,HbA1c,BMD in lumbar spine and hip,OP prevalence among different duration groups(P0.05).② Multiple stepwise regression analysis showed that the major factors affecting BMD of lumbar spine and hip were age,BMI,HbA1c,and menopausal years in postmenopausal women with T2DM.Duration of diabetes and complications of T2DM were not independent factors of the BMD in lumbar spine and hip.Conclusion: BMD and the OP prevalence of T2DM patients were not found related to the state of diabetes complications such as DN,DR and DPN.
