先天性心脏病并肺动脉高压患儿肺动脉压力与血浆血管紧张素Ⅱ水平的关系 被引量:4

摘要 目的观察先天性心脏病(CHD)并肺动脉高压患儿围术期血浆血管紧张素Ⅱ变化,探讨肺动脉压力与血浆血管紧张素Ⅱ的关系。方法选择40例左向右分流型CHD患儿,根据其静息状态下的肺动脉平均压(PAMP)分为4组,无肺动脉高压组、轻度肺动脉高压组、中度肺动脉高压组、重度肺动脉高压组各10例。术前、术后24 h、康复出院时检测各组的血浆血管紧张素Ⅱ,并行超声心动图检查估测PAMP;对肺动脉压与血浆血管紧张素Ⅱ的相关性行线性相关回归分析。结果康复出院时各组PAMP及血浆血管紧张素Ⅱ比较P均>0.05;轻度肺动脉高压组术前、康复出院时PAMP及血浆血管紧张素Ⅱ比较P均<0.05,中、重度肺动脉高压组术前、术后24 h、康复出院时比较P均<0.05。CHD患儿的PAMP与血浆血管紧张素Ⅱ水平呈正相关(r=0.768,P<0.05)。结论CHD并肺动脉高压患儿的PAMP与血浆血管紧张素Ⅱ水平呈正相关。
