经阴道彩色多普勒超声诊断妇科急腹症的应用价值

Application value of transvaginal color Dopper sonography in diagnosis of gynecologic acute abdomen

目的:探讨经阴道彩色多普勒超声在妇科急腹症中的诊断及鉴别诊断价值。方法:回顾性分析176例妇科急腹症的阴道彩色多普勒超声诊断结果并与临床诊断结果对照。结果:本组176例,诊断符合率为86.9%。其中,异位妊娠65例,正确诊断率为90.7%;卵巢囊肿破裂22例,正确诊断率为81.8%;卵巢囊肿蒂扭转25例,正确诊断率为84%;急性盆腔炎36例,正确诊断率为88.8%;宫内孕不全流产28例,正确诊断率为82.1%。结论:经阴道彩色多普勒超声检查对妇科急腹症的诊断正确率较高,且方便、快捷,但当症状不典型,声像图缺乏特异性改变时,容易误诊及漏诊。因此必须详细询问病史,密切结合临床症状、实验室结果,以提高诊断正确率。

Objective： To evaluate values of transvaginal color Dopper sonography in diagnosis and differential diagnosis of gy- necologic acute abdomen. Methods： Diagnosis results of transvaginal color Dopper sonography for 176 cases with gynecologic acute ab- domen were retrospectively analyzed and compared with the clinical results. Results： For 176 cases in the group, the coincidence rate of the diagnosis was 86： 9%, wherein the correct diagnosis rates for 65 cases with ectopic pregnancy, 22 cases with ovarian cyst rup- ture, 25 cases with ovarian cyst torsion ,36 cases with acute pelvic inflammation, and 28 eases with incomplete abortion of intrauterine pregnancy were 90.7 % ,81.8%, 84%, 88.8 %, 82.1%, respectively. Conclusions： Transvaginal color Dopper sonography in diagnosis of gynecologic acute abdomen has a high accuracy and is quick and convenient. But when the symptoms are not typical and the sono- gram lacks of specificity changes, wrong and missed diagnosis happens. So, it is necessary to ask medical history and combine with clinical symptoms and experimental results closely to improve the correct diagnosis rate.
