个案化管理对非典型抗精神病药物所致体重增加的干预作用

Intervention of case management on weight gain caused by atypical antipsychotics

目的:探讨个案化管理对非典型抗精神病药物所致体重增加的干预作用。方法:选择2010年6月1日～2011年5月31日上海市闸北区精神卫生中心住院患者中符合CCMD-3诊断标准的精神分裂症患者,服用单一非典型APS 8周以上,与入院体重相比>7%,共108例,按入院先后随机分为两组。对照组实施常规护理,干预组在常规护理基础上实施个案化管理。在干预前、干预中和干预后第3、6、9、12个月分别进行体重及血清瘦素测评分析。结果:干预组患者体质指数及血清瘦素值得到有效改善和控制,两组前后对照差异有显著性,干预组优于对照组。结论:实施个案化管理能有效控制非典型抗精神病药物所致的体重增加。

Objective：To investigate intervention of case management on weight gain caused by atypical antipsycbotics. Meth- ods：From June 1,2010 to May 31,2011, 108 schizophrenic patients in the hospital meeting CCMD -3 diagnostic criteria, taking sin- gle atypical APS for over 8 weeks, and with a ratio of over 7% of present body weight to that when admitted were randomly divided into experimental group and control group based on the admission time. Routine nursing was given to the patients in control group; whereas those in experimental group received the case management besides the routine nursing. The body weights and serum leptin before, dur- ing, and 3, 6, 9, and 12 months after the intervention were tested. Results： The body mass index and serum leptin of experimental group were effectively improved and controlled, differences of the two groups were significant before and after the intervention, and ex- perimental group was better than control group. Conclusions： The implementation of the case management can effectively control the weight gain caused by atypical antipsychotics.
