彩超在诊断老年2型糖尿病患者下肢动脉阻塞性病变中的应用

Value of color Doppler ultrasound in diagnosis of elderly type 2 diabetic patients with lower extremity arterial occlusive disease

目的探讨彩超在诊断老年2型糖尿病下肢动脉阻塞性病变中的应用价值。方法选择老年2型糖尿病下肢动脉阻塞性病变98例为观察组,97名健康志愿者为对照组,均接受双下肢动脉彩超检查。测量并比较两组间下肢动脉血管管径狭窄、管腔闭塞、管壁斑块及血流量情况。结果①观察组中,彩色多普勒超声共显示下肢动脉斑块、管腔狭窄及管腔闭塞的比例分别为62.24%、74.49%和24.49%,显著高于对照组(P<0.01)。②观察组患者股动脉、腘动脉及足背动脉血管内径分别为(6.32±0.55)mm、(4.97±0.82)mm及(1.56±0.32)mm,均明显窄于对照组(P<0.01)。其股动脉、腘动脉及足背动脉血流量分别为(37.9±7.2)mm3/s、(11.8±2.8)mm3/s和(0.9±0.4)mm3/s,均明显低于对照组(P<0.01)。结论在2型糖尿病并发下肢动脉血管病变的诊断中,彩超具有能够准确的评价动脉管壁、管腔及血流动力学情况,具有较高的应用价值。

Objective To investigate the value of color Doppler ultrasound in the diagnosis of elderly type 2 diabetic lower extremity arterial obstructive lesions. Methods Totally 98 cases of elderly type 2 diabetic lower extremity arterial occlusive disease in the observation group,97 healthy volunteers as control group ,underwent color Doppler ultrasound examination of lower limbs, between lower extremity arterial diameter stenosis and occlusion of the lumen, wall plaques and blood flows were compared. Results In observation group, color Doppler ultrasound shows the lower extremity arterial plaque, stenosis and obliteration rates were 62. 24% ,74, 49% and 24. 49%, significantly higher than the control （P 〈 0. 01 ）. The patients femoral artery,popliteal artery and dorsalis pedis artery vessel diameter （6. 32 ±0. 55） ram, （4. 97 ±0. 82） mm and （ 1.56 ± 0. 32） mm, significantly narrower than the control （P 〈 0. 01 ）. Femoral artery, popliteal artery and dorsalis pedis arterial blood flow （37.9 ±7.2） mm3/s,respectively （11.8±2. 8） rnm3/s and （0. 9 ±0. 4） mm3/s,significantly lower than the control group （ P 〈 0. 01 ）. Conclusions Inthe diagnosis of lower extremity arterial lesions of type 2 diabetes mellitus, color Doppler ultrasound has the ability to accurately evaluate the arterial wall,lumen, and hemodynamic parameters,with a higher value.
