评估SPECT测定不同年龄段健康人肾小球滤过率的准确性 被引量:2

Estimate the accuracy of SPECT determination glomerular filtration rate in different age healthy people

摘要 目的:评估SPECT测定不同年龄段健康人肾小球滤过率(GFR)的准确性。方法:以232例北京地区社区健康居民为研究对象,分别采用双血浆法和99Tcm-二亚乙基三胺五乙酸(99Tcm-DTPA)肾动态显像法测定肾GFR(GFRdt和GFRGates)。以GFRdt为参照标准,分析比较SPECT测定20~29岁、30~39岁、40~49岁、50~59岁、60~69岁及>70岁的6个不同年龄组健康人肾小球率过滤的准确性。结果:①6个不同年龄组所得GFRGates与GFRdt均具有显著性差异。②6个不同年龄组所得GFRGates均明显低于GFRdt,其中30~39岁年龄段GFRGates与GFRdt的差异最大,差异均值为(-34.26±20.79);>70岁年龄组GFRGates与GFRdt的差异最小,差异均值为(-9.48±10.85)。③30~39岁组GFRGates与GFRdt相关性(r=0.42)最差;>70岁组GFRGates与GFRdt相关性(r=0.81)最好。结论:SPECT测定20~49岁健康人GFR的准确性较差,测定≥50岁健康人GFR的准确性较好,其中≥70组准确性最好。 Objective: To estimate the accuracy of SPECT determination glomerular filtration rate in different age healthy groups. Methods: Totally 232 Beijingcommunity health residents were selected as the research subjects. Two-sample method and "Tc'"-DTPA dynamic renal imaging were usea Ior determination of GFR (GFRdt and GFR6ates, respectively). GFRdt as the reference standard, compare the accuracy of six different age groups GFR^ate~ which get by SPECT. Results: ~There were significant differences between GFRdt and GFRGat~ on six different age groups. ~GFRG,,~ from Six different age groups were significantly lower than those of GFRdt. The difference in the group of 50-59years between GFRdt and GFRGat~ is maximum, the mean difference is (-54.26_-_20.79) ml/(mino 1.75m)2. The difference in the group of _〉70years between GFRat and GFR6ates is minimum, the mean difference is (-25.62_+ 18.60) ml/(min* 1.75m)2. ~GFRdt and GFR6,tes derived from the group 50-39years had the worst correlation (r=0.42). GFRdt and GFR^ate~ derived from the group -~70years had the best correlation (r=0.81). Conclusion: The accuracy of GFR which got by SPECT is poor in 20-49years health group, it is better in _〉50 Years health group, while it is the best in 〉-70 years health group.
