心理护理对风心病瓣膜置换手术患者术前手术应激的影响

Influence of Psychological Nursing for Preoperative Surgical Stress of Patients with Rheumatic Heart Valve Replacement

目的探讨心理护理对风心病瓣膜置换手术患者术前手术应激的影响。方法将我院2010年1月至2013年1月间60例风心病患者随机分为研究组和对照组各30例,在常规护理的基础上,研究组采用心理护理,对照组不应用心理护理,比较两组患者术前心理和生理应激变化情况。结果两组护理前心理和生理应激差异无统计学意义(P>0.05),护理后均有明显改善,研究组焦虑改善情况明显优于对照组(P<0.01);研究组患者血糖和血浆皮质醇含量明显低于对照组(P<0.05)。结论心理护理可有效预防和减轻风心病瓣膜置换手术患者术前的心理和生理应激反应,降低手术危险性。

Objective To explore the influence of psychological nursing for preoperative surgical stress of patients with rheumatic heart valve replacement. Methods 60 patients with rheumatic heart disease in our hospital from January 2010 to January 2013 were randomly divided into the study group and the control group, with 30 cases in each group. On the basis of routine nursing, the patients in the study group were given with psychological nursing, while the patients in the control group were not given with psychological nursing. The preoperative psychological and physiological stress changes of the two groups were compared. Results No significant difference before nursing had between the two groups （P〉0.05）, but both groups were significantly improved after nursing, and the improvement of anxiety in the study group was better than that of the control group （P〈0.01）. The blood glucose and plasma cortisol levels of the study group were significantly lower than those of the eontrol group （P 〈0.05）. Conclusions Psychological nursing is effective in preventing and relieving the preoperative psychological and physiological stress response in rheumatic heart disease patients with heart valve replacement and in reducing the surgical risk.
