摘要
目的:应用彩色多普勒超声技术对比房间隔继发孔缺损封堵前后心腔的大小及左室收缩功能的变化。方法:抽取2009年1月-2011年8月在我院行房间隔封堵术的患者资料共35例,分别在术前2d、术后3d、术后1、3和6个月对患者进行常规超声多普勒心动图检查,观察右房横径(RAD1)、右房长径(RAD2)、右室前后径(RVD1)、右室横径(RVD2)、左房前后径(LAD1)、左房横径(LAD2)、左房长径(LAD3)、左室舒张末前后径(LVD1)及左室横径(LVD2)、左室射血分数(EF%)、短轴缩短率(FS%)、每搏量(SV)的变化。结果:与术前比较,术后3d、术后1、3和6个月RAD1、RAD2、RVD1、RVD2逐渐减小,LAD1、LAD2、LAD3、LVD1及LVD2逐渐恢复正常大小,EF%、FS%、SV逐渐增加,以术后3d、术后1及3个月为著。结论:房间隔缺损封堵术作为一种微创治疗先天性心脏病的手术方法,不仅可以纠正错误的血流动力学,而且可以有效的使术后各心腔大小及左室收缩功能恢复正常。
Objective:To study the changes of heart chambers and left ventricular systolic function after transcatheter closure of atrial septal defect by using color Dopple ultrasound. Methods: 35 cases of transcatheter closure of atrial septal de- fect from January 2009 to August 2011 were surveyed. Echocardiography was performed 2 days before operation and 3d, l, 3 and 6 months after operation. Changes in right atrium cross diameter (RAD1) ,right atrium long diameter (RAD2), right ventricular anteroposterior diameter (RVD1), right ventricular cross diameter (RVD2), left atrium anteroposterior diameter (LAD1), left artium cross diameter (LAD2), and left atrium long diameter (LAD3), and left ventricular diastole anteropos- terior diameter (LVD1) and left ventricle cross diameter (LVD2),left ventricular ejection fraction (EF%),left ventricular fractional shortening (FSM) and left ventricular stroke volume (SV) were observed. Results:Compared with those before operation, RAD1, RAD2, RVD1, RVD2 gradually decreased, LAD1, I.AD2, LAD3, LVD1 and LVD2 gradually returned to normal size, EF%, FS%, SV gradually increased, especially after three days ,one month and three months. Conclusion: Tran- scatheter closure of atrial septal defect as a minimally invasive surgical methods of treatment of congenital heart diseases, not only correct the error to blood flow,but also can be effective for heart chambers and left ventricular systolic function re- turning to normal.
