电视胸腔镜在诊治急诊外伤性血气胸中的应用

The application of video-assisted thoracosopic surgery in the diagnosis and treatment of emergent traumatic hemopneumothorax

目的:探讨电视胸腔镜手术(VATS)在诊治急诊外伤性血气胸中的价值。方法:运用电视胸腔镜对31例胸部外伤所致的肋间血管破裂、肺组织裂伤、胸腔内血管引起的血气胸患者进行探查、止血、修补。结果:31例手术全部成功,无手术死亡及并发症,手术时间27～101 min,胸腔引流1～3 d,住院4～10 d。结论:严格把握手术适应证,VATS是诊治急诊外伤性血气胸的一种安全、有效、微创的方法。

Objective： To evaluate the value of video-assisted thoracoscopic surgery（VATS） in the diagnosis and treatment of emergent traumatic hemopneumothorax.Methods： Thirty-one cases with traumatic hemopneumothorax caused by intercostal vascular rupture and lung tissue laceration were probed,stopped bleeding and repaired by VATS.Results： The operation of 31 patients were successful,and no fatality and complications occured.The time of operation,pleural cavity drainage and hospitalization were 27 101 minutes,1-3 days and 4-10 days,respectively.Conclusions： VATS is a safe,effective and less invasive method to diagnose and treat traumatic hemopneumothorax,but surgical indications should be strictly controlled.
