235例危重艾滋病患者临床急救体会

The Clinical Exploring of 235 Emergency Treatment Serious AIDS Patients

目的探讨危重艾滋病患者的临床特点及急诊救治流程和管理,以提高危重艾滋病患者的识别及抢救成功率,实现安全救治。方法收集急诊抢救的235例危重艾滋病患者的临床资料,对其临床特点进行回顾性分析,并于2012年实施了系统的急救管理,其有效性与未实施系统管理进行对比。结果艾滋病病情复杂多样,常因为呼吸衰竭、休克、意识改变、出血等被送入急诊科;收入急诊科的艾滋病患者大多为晚期患者,CD4+T淋巴细胞数多低于50个/μl,常出现多个部位的症状。实施系统的管理方法,提高了危重患者抢救成功率,职业暴露减少,污染物品处理和细菌学监测全部合格,减少了医院感染和职业感染发生的风险。结论晚期艾滋病症状复杂多样,临床应加强识别;建立健全危重艾滋病急救规范,是提高医疗质量,实现安全有效救治的重要因素。

Objective To improve the identification and success rate of rescuing serious AIDS patients and achieve the safety treatment by exploring the clinical traits and emergency rescue process and control of critical AIDS pa- tients. Methods We collected clinical datas of 235 serious AIDS patients treated with emergency services and made a retrospective analysis of the clinical traits. After carrying out systematic administration in 2012, we com- pared with effectivity before and after carrying this systematic administration process. Results The condition of AIDS patients is complex and diverse. AIDS patients were sent into emergency department often because of respira- tory failure, shock, altered consciousness, bleeding, and so on, whats more, they were late stage of disease with the number of CD4＋ T lymphocyte below 50μ1 in blood and emerged multiple focal symptoms constantly. After carry- ing out systematic administration process, we realized the improvment of success rate of rescuing emergency and se- rious patients,reduction of occupation exposure, all qualified rate of Pollutant treatment and bacterial monitoring and reducing the risk of occupational infection and nosocomial infection. Conclusion The symptoms of advanced AIDS patients are complex and diverse, we should enhance the capacity of identifying this disease in clinical works; it is important for establishing and improving standard of serions AIDS patients treated with emergency services to improve the quality of medical care and realize safe and effective treatment.
