三种腭裂术式的术后腭咽功能评估

Postoperative evaluation of velopharyngeal function by three different cleft repairs

目的通过对腭裂术后腭咽功能的评估,比较软腭直线缝合、Furlow术式和Sommerlad术式的优、缺点。方法对248例年龄5个月至11岁的先天性腭裂患者分别采用软腭直线缝合、Furlow术式和Sommerlad术式,并观察、比较腭裂术后并发症、术区瘢痕、腭咽运动、语音等指标。结果除术后并发症、咽反射水平三组无明显差异外,在术后瘢痕、头颅定位侧位片测量、鼻漏气、语音清晰度等指标的评估结果,Furlow术式和Sommerlad术式均优于软腭直线缝合法。结论 Furlow术式和Sommerlad术式在恢复腭咽闭合功能方面优于软腭直线缝合法。

Objective To compare the advantage and the shortage of straight line, Furlow and Sommerlad repair of soft palate, by postoperative evaluation of velopharyngeal function. Methods 248 patients, from 5 months to 11 years old, were treated in our department by straight line, Furlow and Sommerlad repair of soft palate respectively. Postoperative complications, scars, velopharyngeal movement and articulation of those patients were observed and evaluated. Results There was no significant difference among three groups on postoperative complications and pharyngeal reflection. But Furlow and Sommerlad repair are better than straight line repair of soft palate on evaluation of scars, X-ray velopharyngeal lateral radiographs, nasal leaking and speech intelligibility. Conclusion Furlow and Sommerlad repair are better than that of straight line repair of soft palate to recovery the function of velopharygeal closure.
