中药四磨汤对婴幼儿先心病术后胃肠功能恢复的影响

Effects of Simo Decoction on postoperative recovery of gastrointestinal function in infants with congenital heart disease

目的探讨四磨汤对婴幼儿先心病术后胃肠功能恢复的影响。方法将2010年1月～2012年6月在我院行体外循环心脏直视手术的婴幼儿先天性心脏病316例分为实验组和对照组,在拔除气管插管后2 h,实验组(n=152)即给予中药四磨汤口服液1.5 ml/kg,3次/d,连续使用3 d;对照组(n=164)给予等量温开水口服,其他处理两组无异。观察两组术后肠鸣音恢复时间、首次排便时间,恶心、呕吐、腹胀发生率,并做胃电图检测。结果两组术后均出现一定比例的恶心、呕吐、腹胀,但实验组发生率明显低于对照组(P<0.05),分别为11.84%(18/152)vs 20.73%(34/164);实验组术后肠鸣音恢复时间明显短于对照组[(27.38±3.19)vs(44.83±3.66)h,P<0.05];实验组首次排便时间也明显短于对照组[(47.52±4.63)vs(57.04±4.43)h,P<0.05];实验组在术后各个检测时限点上,胃电振幅和慢波百分比均高于对照组(P<0.05)。结论中药四磨汤可以促进婴幼儿先心病术后胃肠道功能的恢复。

Objective To discuss the effects of Simo Decoction on postoperative recovery of gastrointestinal function in infants with congenital heart disease.Methods The 316 infants with congenital heart disease,who received the cardiac operation under direct vision with extracorporeal circulation in our hospital from January 2010 to June 2012,were randomly divided into experimental and control group.The experimental group（n = 152） received Simo Decoction oral liquid 1.5 ml / kg 2 hours after the extubation for 3 times a day and continuous 3 days.The control group（n = 164） only orally took the same volume of warm water instead.Observation was made in the recovery time of bowel sound,the first defecation time,and the incidence of nausea,vomiting,and abdominal distension.And also electrogastrogram was carried out.Results There was a certain percentage of infants with nausea,vomiting,and abdominal distension in the two groups after the operation,but the incidence of the experimental group 11.84%（18 /152） was significantly lower than that of the control group 20.73%（34 /164）（P 0.05）.The recovery time of bowel sound in the experimental group was significantly shorter than that in the control group[（27.38 ± 3.19） vs（44.83 ± 3.66） h,P 0.05].The first defecation time in the experimental group was also significantly shorter than that in the control group[（47.52 ± 4.63） vs（57.04 ± 4.43） h,P 0.05].At all postoperative detection time limit points,the gastric electrical amplitude and slow wave percentage in experimental group were all higher than those in the control group（P 0.05）.Conclusion Simo Decoction can promote the postoperative recovery of gastrointestinal tract function in infants with congenital heart disease.
