摘要 目的 研究负压对创面局部血流量的影响 .方法 采用猪的新鲜和肉芽创面模型 ,以特制的闭合敷料封闭创面 ,负压装置调控创面微环境负压 ,用激光多普勒血流仪连续测定负压前后两种创面血流量的动态变化 .结果 新鲜创面 :8k Pa负压时创面血流量即明显升高 ;16 k Pa负压时其峰值接近基线血流量的 4倍 ,持续负压 30 min后维持在基线水平的2倍左右 ;2 0 k Pa负压以上时血流量增加后很快降至基线水平 ;16 k Pa间歇性负压时血流量曲线呈规律的方形波状变化 .肉芽创面 :16 k Pa负压下血流量峰值同新鲜创面 ,接近基线血流量的 4倍 ,但负压持续 15 m in血流量下降不显 . AIM To study the effect of negative pressure on local wound blood flow. METHODS A pig acute and granulation wound model was established. A special occlusive dressing was applied to seal the wound, and negative pressure was applied and transmitted to the wound in a controlled manner. Blood flow was recorded continuously before and after the application of negative pressure on Laser Doppler flowmeter. RESULTS In the acute wound, blood flow increased when the negative pressure was at the level of 8 kPa. The peak blood flow recorded at 16 kPa negative pressure was up to 4 times the baseline blood flow. Blood flow remained around 2 times the baseline values after 30 minutes ofcontinuous application of negative pressure. The increase declined and reached the baseline soon after the application of negative pressure at the level of 18 kPa and above. Intermittent application of negative pressure resulted in blood flow that fit a square wave pattern. In the granulation wound, the peak blood flow recorded at 16 kPa negative pressure was about 4 times the baseline values. However, the increases didn't decline significantly after 15 minutes of continuous application of negative pressure. CONCLUSION It can be concluded that negative pressure can increase blood flow significantly.
