摘要
目的用有限元分析方法对横形髋臼骨折模型进行前柱、后柱、双柱3种螺钉内固定置入方法静力有限元分析比较,为临床选择更理想的内固定方法提供理论依据。方法以正常骨盆螺旋CT扫描的基础上,用Mimcs 10.0分割出右侧髋骨点云图数据,再用Solidworks 2010制作成实体和模拟髋臼横形骨折,进行前后柱螺钉内固定,将各项零件组装成手术装配体模型,直接导入Ansys 12.1进行无骨折和3种内固定方法分别进行位移和应力2项静力分析。结果通过位移和应力云图分析与比较后发现,采用双柱螺钉内固定总位移最小,髋臼部应力分布与无骨折模型相似,应力集中在前柱螺钉上。单柱固定位移均有增大和螺钉部应力集中表现,后柱固定髋臼位移和螺钉应力集中均小于前柱固定。结论横形髋臼骨折双柱固定优于单柱固定,后柱固定优于前柱固定。
Objective To analyze and compare three screw internal fixation methods, anterior column, posterior column and double column for acetabular fracture model with finite element method and provide experimental evidence for choice of suitable clinical internal fL^ation. Methods In a normal pelvic spiral CT scans, and on the basis of Mimcsl0.0 with segment the right hip point cloud data, Solidworks2010 was used to make entities and simulation of transverse acetabular fracture, screw internal fixation in double column, all the parts were assembled into surgery assembly body model, direct import Ansys12.1 without fracture and three kinds of internal fixation method respectively displacement and stress two static analysis. Results The displacement and stress analysis and comparison on cloud, found that the screw internal fixation with double column total minimum displacement, stress distribution and the acetabular without fracture model similar, stress concentration in setting the anterior column screws. The fixed displacement was simple columnar displacement increase and screw stress concentration of performance, posterior column after fixed acetabular displacement and stress concentration screws were less fixed setting after anterior column. Conclusion Among these internal fixation methods in treating transverse acetabular fracture, double column fLxation is better than simple column, and posterior column fixation is better than anterior column.
