摘要 目的 :研究内皮素 (ET)受体基因亚型在心功能不全心肌中的表达 ,并探讨它和心功能分级及心肌肥大的关系。  方法 :选取 40例行瓣膜置换术的心功能不全病人心肌 5 0 0 mg,同时取 10例健康心肌组织作为对照。提取组织总核糖核酸并逆转录为互补脱氧核糖核酸后 ,加入特定的寡聚核苷酸引物进行聚合酶链反应 ,分别观察 ET不同受体基因的表达。  结果 :ET受体基因在正常心肌组织中未见表达 ,而在病变心肌组织中的表达较高。  结论 :心功能不全时血浆 ET水平升高 ,其受体并不呈反应性下调 ,而是表达增多 ,并与左心收缩功能下降程度有关。 Objective:To investigate the expression level of endothelin receptor subtype messenger ribonucleic acid(mRNA)in myocardial dysfunction. Methods:The specimens came from patients suffered with congestive heart failure who had been hospitalized for operation.Endothelin(ET)A and ETB gene expression is detected in reverse transcriptase polymerase chain reaction ( RTPCR ) methods.The hypertrophic degree of left ventricular myocardium was assessed by Echocardiography. Results:ETA gene expressed in 27/40 biopsies(67 5%)in patient group and none in control group.ETB gene expressed in 20 biopsies(50%)and none in control group.ETA expressed 40% in NYHA class Ⅰ,53 3% in class Ⅱ,84 6% in class Ⅲ and 85 7% in class Ⅳ,whereas ETB expressed 20% in NYHA class Ⅰ,33 3% in class Ⅱ,69 2% in class Ⅲ and 71 4% in class Ⅳ. Conclusion:ETA and ETB gene expression are both upregulated in failing human cardial myocytes and correlated with NYHA class.The ET system may play a key role in the pathophysiological process of myocardial dysfunotion in autocrine/paracrine.
