支气管激发试验联合血清IgE检测对咳嗽变异型哮喘的诊断价值

Role of bronchial provocation test associated with serum IgE measurement in diagnosis of cough variant asthma

目的探讨支气管激发试验联合激发前后血清TIgE检测对咳嗽变异型哮喘（CVA）诊断的特异性和敏感性。方法将咳嗽变异型哮喘与急性支气管炎和无支气管疾病的患者进行比较，分别进行检测FEV_1／FVC、支气管激发试验，以化学发光法分别检测每例患者支气管激发试验前后的TIgE水平。结果CVA组气道高反应性的阳性率为96．7％，显著高于急性支气管炎组和对照组。CVA患者激发前和激发后的TgE水平分别为（150.25±52．71）Ku／ L和（261．22±27.44）Ku／L，与急性支气管炎组和对照组比较差异均有显著性（P＜0．05）；TIgE的阳性率比较差异亦有显著性（P＜0．05）。结论对怀疑有咳嗽变异型哮喘的病人同时行气道高反应性和TIgE水平检测，具有较高的诊断特异性。

Objective To investigate the specificity and sensibility of bronchia1 provocationtest associated with serum T IgE level measurement in the diagnosis of cough variant asthma (CVA).Methods All subjects were divided into 3 groups, including CVA, acute bronchitis (AB) and controlgroup. FEV_1/FVC, Serum T IgE level in all subjects were measured before and after bronchialprovocation test. Results The positive rate of bronchial hyperreactivity (BHR) and serum T IgElevel in CVA group was significantly higher than those in other groups (P <0.05). ConclusionBronchial provocation test associated with T IgE level measurement in diagnosis of CVA have veryhigh diagnostic specificity.
