浆细胞性乳腺炎的超声诊断

The ultrasonic diagnosis of plasma cell mastitis

目的 探讨浆细胞性乳腺炎超声诊断价值。方法 采用高频探头病侧乳房全方位探测。结果 31例浆细胞性乳腺炎的超声诊断与术后病理诊断符合率 5 1 6 % (16 / 31) ,误诊为乳腺癌19 4% (6 / 31) ,未作出明确诊断 2 9% (9/ 31)。其声像图表现为 :低回声型 (10例 ) ;多囊型 (6例 ) ;混合型 (14例 ) ;强回声型 (1例 )。 8例进行了彩色多普勒超声检查 ,发现肿块内有少量散在血流 6例。此病与乳腺癌在声像上的不同点是 :1.肿块内有无囊状小暗区、粗斑点和条索状光带。 2 .后方有无声衰减 ,与胸大肌有无粘连。 3.彩超检查肿块内有无丰富的血流信号。结论 高频超声加以彩超辅助对此病的诊断有一定的实用价值。

Objective To discuss the value of ultrasound in the diagnosis of plasma cell mastitis.Methods The patient′s breast was examined in all direction by B-us high frequency transducers.Results The coincidence rate of the diagnoses of plasma cell mastitis between B-us and post-operative pathological examination was 51 6%.Of 31 cases,six cases were misdiagnosed as breast cancer(19 4%),and a definite diagnosis failed to be established in nine cases (29%).The acoustic patterns of the disease exhibited as follows:1.hypoechoic texture type(in 10 cases);2.polycystic type (in 6 cases);3.mixed type (in 14 cases);4.hyperechoic texture type(in one case).Color Doppler sonography was conducted in eight cases,and a few of sporadic blood flow signals were detected in the mass in six of the eight cases.The difference of the acoustic patterns between plasma cell mastitis and breast cancer was analysed and discussed,which indicated as follows:1.whether small cystic anechoic areas,echogenic lines and echogenic spots could be found in the mass or not;2.whether the echogenic focuses could gradually attenuate or not,and adhesion to greater pectoral muscle could be detected or not;3.under the examination of color Doppler sonography,whether abundant blood flow signals could be viewed in the mass or not.Conclusion The B-mode ultrasound plus color Doppler sonography diagnosis of plasma cell mastitis is of practical values.
