摘要
目的 :揭示超声在乳癌诊断中的价值 ,提高诊断水平。方法 :分析 1997年 1月至 2 0 0 0年4月间 74例经手术病理或穿刺活检证实为乳癌患者的高频声像图和彩色多普勒特征。结果与结论 :常见恶性乳腺肿块的高频声像图特征 :形态不规则 ,边界不清 ,回声不均 ,多为低回声 ,部分伴有钙化灶 ,后方衰减 ,中、晚期病例可见周边小病灶和同侧腋窝甚至锁骨上淋巴结转移。在应用彩色多普勒观察时 ,可见肿瘤内的血管明显增多 ,且肿瘤越大血流信号越丰富。对个别超声印象不典型的病例或为便于制定手术方案 ,缩短患者手术时间 ,可实施超声引导下穿刺活检。
Objective: To demonstrate the important role of high frequency ultrasonography (HFU) and color Doppler ultrasonography (CDU) in the diagnosis of breast carcinoma in order to improve the level of diagnosis. Methods:The common characteristics of HFU and CDU performed in 74 patients with breast carcinoma from January 1997 to April 2000 were analyzed. Results and conclusions: On HFU, the common characteristics of malignant breast masses were as follows: irregular morphology, indistinctly defined border, inhomogeneous echogenicity, distal acoustic shadow, small peripheral focus of disease in some cases, as well as metastatic tumors in axillary and supraclavicular lymph nodes. On CDU, it could be seen that the blood vessels within tumor masses increased evidently in number and the tumor size was closely related with the sonogram of blood flow. The ultrasound-guided automatic biopsy can play an important role in the diagnosis of breast carcinoma and the scheme-making of surgical operation. [
