Ⅰ、Ⅱ_a期乳腺癌哨兵淋巴结活检术的临床应用意义

Clinical Application of the Sentinal Node Biopsy in Stage Ⅰ/Ⅱ_a Breast Cancer

目的 探讨哨兵淋巴结活检术在Ⅰ、Ⅱa 期乳腺癌手术治疗中的应用价值。方法 选择Ⅰ、Ⅱa 期乳腺癌 33例 ,采用99mTc DX为示踪剂行哨兵淋巴结显像检查 ,并作好体表标记 ,术中先行哨兵淋巴结活检 ,再行乳腺癌根治性手术 ,根据术后病理检查结果进行分析。结果 30例哨兵淋巴结显像 ,显示率达 90 .9% (30 / 33) ,其中活检阳性 12例 (腋窝淋巴结清扫标本中 ,有 11例淋巴结阳性 ) ,在哨兵淋巴结阴性的 18例中 ,腋窝淋巴结阳性 2例 ,预测腋窝淋巴结状况正确率为 90 .0 % (2 7/ 30 )。结论 乳腺癌哨兵淋巴结活检术是 1项有实用价值的新技术 ,它能较为正确地反映腋窝淋巴结的状况 ,且创伤小 ,预测淋巴结转移的正确率高。

Objective To assess the clinical valus of sentinal node(SLN) biopsy in Ⅰ/Ⅱ a breast cancer.Methods 33 cases of stage Ⅰ/Ⅱ a breast cancer were enrolled. 99m Tc-DX technique was used to develop the SLN,and the skin of body was labeled.During surgery,the SLN biopsy was done before the radical mastectomy.Results 30 cases had SLN development,video rate was 91%(30/33).12 cases had SLN positive and 11 cases were found positive node in axillary lymph node(ALN) biopsy.There were two cases ALN positive in 18 cases SLN negative.SLN prediction accuracy rate was 90%(27/30).Conclusion SLN idcntification has clinical valus for early breast cacer.It is a new technology,which gives small injury and high accuracy in prediction of ALN metastasis.
