摘要
目的:采用组织运动瓣环位移技术(TMAD)定量分析慢性心力衰竭(CHF)患者左室整体与节段纵向收缩功能,探讨其应用价值。方法:选择CHF患者22例,同期选择健康人20例作为对照组。采用QLAB8.1软件测量左心室整体与节段纵向应变(LSR),测量二尖瓣环6个位点(前间隔侧,后间隔侧,前壁侧,侧壁侧,后壁侧以及下壁侧)平均值,代表二尖瓣环整体位移(TMAD)。比较两组TMAD、LSR等参数,并分析LSR与TMAD的相关性。结果:与对照组比较,CHF组二尖瓣环6个位点TMAD以及TMADglobal均低于对照组(P<0.05);与对照组比较,CHF组各壁LSR以及GLS均低于对照组(P<0.05);两组|GLS|与TMADglobal分别呈线性正相关,两组|GLS|以及TMADglobal分别与LVEF呈线性正相关。结论:TMAD、LSR与LVEF具有相关性,是评价CHF患者左心室纵向整体与节段收缩功能的新方法。
Objective: To assess the left ventricular global and segmental longitudinal systolic function in patients with chronic heart failure (CHF) using tissue mitral annulus displacement (TMAD) quantitative analysis. Methods: There were 22 CHF patients and 20 healthy volunteers matched by gender and age. Left ventricular longitudinal strain (LSR) and the mitral annular displacement (six sites: anterioseptal, posterioseptal, anterior, lateral, posterior and inferior sites respectively ) were measured by the technique of 2D Strain Imaging (2D-SI) with QLABS. 1 software. The parameters of LSR and TMAD from two groups was compared, and the correlation between them was analyzed. Results: Compared with the control group, both TMAD of six spots and TMADglobal of CHF group were significantly decreased ( P 〈 0.05 ), as well as LSR and GLS ( P 〈 0.05 ). There were positive correlation between | GLS| and TMADstobal, as well TMADglobal and | GLS| with LVEF. Conclusion: TMAD and LSR are correlated with LVEF, which is a new method to assess left ventricular global and segmental longitudinal systolic function in patients with CHF.
