摘要
目的:观察补体抑制剂眼镜蛇毒因子(CVF)对脓毒症大鼠心肌组织的保护作用。方法:144只清洁级雄性SD大鼠(体质量250~300 g)被随机分为大鼠假手术组(A组)、脓毒症组(B组)、CVF组(C组)3组,每组48只。A组仅开腹翻动盲肠后关腹并复苏,B组和C组均行经典盲肠结扎穿孔(CLP)制作成脓毒症模型。C组于术前24 h给药,A组和B组则给予等量0.9%氯化钠溶液;各组按照标本采集点的不同又分为术后2、8、16、24 h时间点检测血清补体C3水平、心肌组织髓过氧化物酶(MPO)的变化及观察各组大鼠心肌组织病理学改变。结果:①B组术后血清补体C3浓度最高,C组最低(P<0.05),且一直维持较低水平;②在8、16和24 h时C组MPO活性较B组显著下降(P<0.05),其中以术后16 h最为明显;③C组不同时点心肌损伤均较B组有所减轻,三组不同时间点的差异以术后16 h最为明显。结论:CVF可以明显减少血清补体C3及MPO水平,并有效减轻脓毒症大鼠的心肌损伤,显示出对心肌组织良好的保护作用。
Objective: To observe the protective effects on myocardial tissues by inhibition of complement activation of cobra venom factor (CVF) in septic rats. Methods: One hundred and forty-four SD rats (male, 250~300 g) were randomly divided into three groups (each n=48): group A (sham group): abdomen was opened and closed later but without cecal ligation and puncture (CLP); group B (sepsis group): sepsis was induced by CLP; group C (CVF pretreatment group): sepsis was induced by CLP after pretreatment with CVE At 24 hours before the operation, rats were given a dose of 50 la g/kg CVF in group C or an equal volume of saline solution in both groups A and B. To measure the levels of both complement C3 in serum and MPO, meanwhile the histopathological changes of myocardial tissues were observed. Results: There were significant differences in plasma comple- ment C3 among the three groups; group B after operation was higher than group A and group C (P 〈 0.05), the level in group C was the lowest (P 〈 0.05) and remained low level in all time points. The MPO levels of myocardial tissues in group A remained low in all time points. However, the levels in group B and group C raised quickly after operation and were higher than that of group A significantly (P 〈 0.05). The levels in group C were lower than that in group B significantly, especially in the 16th points (P 〈 0.05). The histopathological changes showed that the myocardial tissues were more severe in group B than that in other groups; Myocardial tissues in group C were eased significantly compared to that of group B. Conclusion: CVF can reduce the levels of both complement C3 in serum and MPO, lessen the myocardial injury in septic rats significantly, which showed the protective effects on myocardial tissues of seotic rats.
