类风湿性关节炎患者心理健康状况与社会人口学因素的关系

Relationship between Social Demography Factors and Mental Health Status of Patients with Rheumatoid Arthritis.

目的探讨社会人口学因素对类风湿性关节炎患者心理健康状况的影响,为进一步干预积累临床资料。方法用一般情况调查表、社会支持量表(SSRS)和症状自评量表(SCL-90)对80例类风湿性关节炎患者进行调查,比较本组患者SCL-90评分与国内常模的差别,分析患者社会人口学资料与SCL-90评分之间的关系。结果本组患者SCL-90评分,除人际关系和精神病性因子外,其余各因子分和总分以及阳性项目数显著高于国内常模(t=10.250,6.848,5.179,6.013,2.914,9.749,2.163,7.679,5.707;P均<0.05);多元回归分析,社会支持、付费类别、经济状况、户籍因素进入SCL-90总分回归方:y=225.682-1.488x1+20.468x2-19.625x3+12.313x4(F=51.756,P=0.000),并能预测SCL-90评分总变异的72%。结论类风湿性关节炎患者心理健康状况较差,社会支持、付费类别、经济状况和户籍是影响患者心理健康的主要社会人口学因素。

Objective To explore the influence of social demography factors on mental health status of patients with rheumatoid arthritis,and accumulate clinical data for further intervention.Methods A total of 80 patients with rheumatoid arthritis were investigated with the questionnaire about general condition and were evaluated with Social Support Rating Scale（SSRS）and Symptom Checklist 90（SCL-90）.To compare and analyze the difference of the scores of SCL-90 between the group and the Chinese norm along with the relationship between the scores of SCL-90 and the social demography data of patients.Results Factors and total scores along with the positive projects of SCL-90 in the group was significantly higher than the Chinese norm except interpersonal sensitivity and psychotic factor（t=10.250,6.848,5.179,6.013,2.914,9.749,2.163,7.679,5.707;all P0.05）.By multiple regression analysis,factors of social support,pay category,financial situation and household register entered into the regression equation：y=225.682-1.488x1＋20.468x2-19.625x3＋12.313x4（F=51.756,P=0.000）.It could forecast 72% variance of SCL-90.Conclusion The mental health status of patients with rheumatoid arthritis is rather bad.Social support,pay category,financial situation and household register are main social demography factors influencing on mental health of patients.
