摘要
目的探讨结核分支杆菌聚合酶链反应(Polymerase chain reaction,PCR)检测对菌阴肺结核的诊断价值。方法对8 2例临床诊断菌阴肺结核患者行痰及纤维支气管镜(支气管肺泡灌洗液Bronchoalveolar lavage fluid,BALF)结核分支杆菌PCR检测。结果痰检结核杆菌PCR检测,其中22例高于正常范围,阳性率26.8%;BALF结核杆菌PCR检测,其中4 6例高于正常范围,阳性率5 6%。BALF结核菌涂片阳性9例,阳性率1 0.9%;其中7例结核杆菌PCR检测高于正常范围,阳性率7 7.8%。结论 PCR作为一种简便快捷的基因扩增技术,用该方法检测结核杆菌显示出很高的灵敏度和特异度,早期快速协助菌阴肺结核的诊断,以便于患者早期强化抗痨治疗,减少误诊及漏诊,消除患者的顾虑。
Objective Investigating the diagnostic value of the mycobacterium tuberculosis PCR detection to the mycobacterium tuberculosis negative pulmonary tuberculosis. Methods Detection the mycobacterium tuberculosis PCR of the sputum and bronchofibroscope bronchoalveolar lavage fluid in 82 patients whose the clinical diagnosis is mycobacterium tuberculosis negative pulmonary tuberculosis. Results The sputum' s PCR value were higher in 22 pations which the positive rate is 26.8% and the BALF's PCR value were higher in 46 pations which the positive rate is 56%.The BALF's tubercle bacillus film preparation was positive in 9 patients which the positive rate is 10.9% and among them 7 patients' BALF's PCR value were higher which the positive rate is 77.8%. Conclusion The method of the gene amplification have the higher sensitivity and specificity in the detection of the tubercle bacillus. The mycobacterium tuberculosisPCR detection to the mycobacterium tuberculosis negative pulmonary tuberculosis is easier and quickly.
