高血压病患者心电图左室肥厚与脑卒中发生的关系 被引量:8

摘要 目的探讨高血压病患者心电图左室肥厚与脑卒中发生的关系。方法入选212例高血压病患者分为两组:高血压并脑卒中组109例,高血压组103例。入选病例均进行常规心电图检查,Sokolow-Lyon电压值>4.0 mV(男)或>3.5 mV(女)诊断为左室肥厚。结果高血压并脑卒中组患者左室肥厚的发生率为30.3%,高血压组左室肥厚的发生率为17.5%,两组间比较有统计学意义(P<0.05)。高血压患者心电图左室电压值与血压水平呈正相关。在高血压患者,心电图左室肥厚显著增加脑卒中发生的风险,OR值为2.283(95%CI=1.145~4.552)(P<0.05)。结论高血压并发脑卒中患者心电图左室肥厚的发生率增高。高血压病患者心电图左室肥厚预示着脑卒中发病的风险增高。 [Objective] To investigate the relationship of electrocardiographic left ventricular hypertrophy (LVH) and stroke in patients with essential hypertension. [Methods] Totally 212 patients with essential hy- pertension were classified into two groups: 109 cases with stroke and 103 cases without stroke. All patients underwent standard electrocardiography. Electrographic LVH was defined as Sokolow-Lyon voltage 〉4.0 mV in men or 〉3.5 mV in women. [Results] The electrocardiographic LVH rate was higher among hypertensive pa- tients with stroke than among those without stroke (30.3% versus 17.5%, P〈0.05). For the hypertensive pa- tients, the values of Sokolow-Lyon voltages were positively correlated with the levels of blood pressure. In patients with essential hypertension, electrocardiographic LVH was associated with increased risk of stroke (OR =2.283, 95% CI =1.145-4.552) (P〈0.05). [Conclusion] The prevalence of electrocardiographic LVH is in- creased in hypertensive patients with stroke. In patients with essential hypertension, LVH diagnosed by elec- trocardiography confers an increase in the risk of stroke.
