肝胆管囊腺瘤的临床表现和影像学诊断

Clinical Manifestations and Imaging Diagnosis of Hepatobiliary Cystadenoma

目的探讨肝胆管囊腺瘤的临床特征和CT表现,以提高该病的术前诊断水平。方法回顾性分析3例经手术切除并经病理证实的肝胆管囊腺瘤的临床表现和CT特点,并结合文献复习。结果 3例肝胆管囊腺瘤患者,男1例,女2例。临床表现为肝区疼痛不适,1例出现发热、黄疸;血清肿瘤标记物检查1例CA199轻度升高。1例病灶位于肝左叶,2例位于肝右叶,均表现为囊实性肿块,实性成分动脉期轻中度强化,静脉期持续强化。结论肝胆管囊腺瘤好发于中年女性,影像学表现有一定的特征性。

Objective To summarize the clinical characteristics and imaging appearances of hepatobiliary cystadenoma and to improve the diagnostic accuracy. Methods Clinical features and imaging findings of 3 cases with hepatobiliary cys-tadenoma proved by pathological and surgery findings were analyzed retrospectively. Results Hepatobiliary cystadenomas were found in 1 male and 2 females. The most common presenting complaint was ＇abdominal pain on the fight upper quad- rant. Fever and jaundice occurred in one case. Serum tumor marker levels were normal except CA199 increased in one case. The tumors located at left lobe in 1 and right lobe in 2 cases. The tumors showed cystic-solid with well defined mar-gins. The solid parts of tumors showed various degrees of enhancement. Conclusion The imaging appearances of hepato-biliary cystadenoma have certain characteristics. Combined with clinical manifestations is helpful to make the correct diag-nosis.
