超声造影在卵巢肿瘤定性诊断中的临床价值 被引量:13

摘要 目的探讨超声造影对卵巢肿瘤定性诊断的应用价值。方法对48例卵巢肿瘤进行经阴道彩色多普勒超声和超声造影检查,并与病理结果对照。结果经阴道彩色多普勒超声诊断卵巢肿瘤良恶性的敏感性88.0%(22/25),特异性87.0%(20/23),准确性87.5%及正确诊断指数75.0%;经腹超声造影诊断卵巢肿瘤良恶性的敏感性96.0%(24/25),特异性95.7%(22/23),准确性95.8%及正确诊断指数91.7%;二者真实性及可靠性比较差异均有统计学意义(均P<0.05)。超声造影对卵巢肿瘤定性诊断与病理诊断一致性较好(Kappa值为0.9165)。结论实时超声造影显示肿瘤的血流灌注信息方面较经阴道彩色多普勒超声更具优势,可用于提高卵巢肿瘤的早期诊断及鉴别诊断准确性。 Objective to explore lhe clinical wdue of conlrasl-enhaneed uhrasonography in tile qualitative diagnosis of ovarian cancer. Methods Transvaginal color Doppler ultrasound and contrasl-ellhanced uhrasonography were performed in 48 patients with ovarian cancer. The resuhs were compared with pathological findings. Results The sensilivity, specificity, aeuraey and Youden's index in the diagnosis of benign and malignant ovarian tumors by contrast-enhanced uhrasonography and transvaginal color Doppler sonography were 96.0%, 95.7%, 95.8%, 91.7% and 88.0%, 87.0%, 87.5%, 75.0%, respectively. Contrast-enhanced uhrasonography had good ccmsistency with lmthologieal examination in Ihe qualitative diagnosis of ovarian lumor (Kappa=0.9165). Conclusion The real-lime contrast-enhanced uhrasonography has more advanlages lhan transvaginal color l)oppler sonography in demonstrating lumor blood perfusion and can be used to improve the accuracy of early diagnosis of ovarian tumors.
