晶状体后囊破裂型眼外伤Ⅱ期人工晶状体植入

Effect of secondary intraocular lens implantation in traumatic cataract with posterior capsular rupture

【摘要】目的探讨晶状体后囊破裂型眼外伤Ⅱ期后房型人工晶状体植入的效果。方法A组：36例（36眼）行外伤性白内障I期摘出联合I期后房型人工晶状体植入；B组：36例（36眼）1～6个月后行醐白内障摘出术联合后房型人工晶状体植入。对术后视力及并发症进行分析比较。结果术后8周视力〉0．5者，A组22眼（61．1％），B组27眼（75．0％），两组差异有统计学意义（x2=8．175，P〈0．05）；术后两组患眼均有不同程度的炎症反应，B组并发症低于A组，（x2=10．125，P〈0．05）。结论Ⅱ期后房型人工晶状体植入手术，安全性高，并发症少，可使患者获得较好的矫正视力。

Objective To explore the effect of secondary intraocular lens （IOL） implantation in traumatic cataract with posterior capsular rupture. Methods 72 cases of traumatic cataract with posterior capsular rupture were divided into two groups： group A （36 eyes, 36 cases） and group B （36 eyes, 36 ca- ses）. Patients in group A were treated by traumatic cataract removal and primary IOL implantation. Patients in group B underwent traumatic cataract removal and secondary IOL implantation. The visual acuities and surgical complications were compared between two groups. Results There were 22 eyes （ 61.6% ） in group A and 27 eyes （75.0%） in group B with visual acuities better than 0.5 at 8 weeks after surgery and the difference was statistically significant （X2 = 8. 175, P 〈 0. 05 ）. The different degrees of inflammatory re- action were observed in two groups. The incidences of surgical complications in group B were much lower than that in group A （X2 = 10. 125, P 〈 0.05 ）. Conclusion The traumatic cataract with posterior capsular rupture treated by secondary IOL implantation can have better visual outcomes and less surgical complications compared with primary IOL implantation.
