单纯内固定与内固定结合椎体成形术治疗胸腰椎骨折的比较

Efficacy comparison of simple pedicle screw fixation and fixation combined with vertebroplasty in treatment of thoracolumbar fracture

目的探讨单纯内固定与内固定结合椎体成形术治疗胸腰椎骨折的临床疗效。方法将66例胸腰椎骨折患者分为两组,对照组30例行单纯内固定,成形组36例行内固定结合椎体成形术,对比两组的临床指标、Frankel分级变化、影像学结果。结果两组术后Frankel分级及生活质量均优于术前(P<0.05),成形组的临床指标、影像学结果均优于对照组(P<0.05)。结论内固定结合椎体成形术治疗胸腰椎骨折,符合生物力学特征,可有效恢复椎体力学性能,临床疗效满意。

Objective To compare the efficacy of simple pedicle screw fixation with fixation combined with vertebro- plasty in treatment of thoraculumbar fracture. Methods 66 patients with thoracolmnbar fracture were randomized di- vided into two groups, 30 patients underwent simple pedicle screw fixation （control group）, 36 patients underwent pedicle screw fixation （treatment group） combined with injectable calcium sulfate cement vertebroplasty. The clinical indexes, neurological status distribution and radiographic parameters were compared between two gruups. Results All patients with neurological function deficits were improved postoperatively, and quality of life were better than pre- operation （P 〈 0. 05）. During follow-up, patients in the treatment group had better results of clinical outcomes and radiographic parameters than in the control group （ P 〈 0. 05 ）. Conclusions Pediele screw fixation combined with injectable ealcimn sulfate cement vertebroplasty is a safe and effective method in treatment of thoracolumbar fracture, which fits biomechanieal features and has the better clinical efficacy than simple pediele screw fixation.
