腓动脉穿支蒂顺行皮瓣修复小腿中上段骨及钢板外露软组织缺损

Application of perforating branches of peroneal artery pedicle antegrade flap to repair upper calf bone and soft tissue defect with plate exposure

目的探讨采用腓动脉穿支蒂顺行皮瓣修复小腿中上段骨及钢板外露、软组织缺损的临床疗效。方法采用腓动脉穿支蒂顺行皮瓣修复9例胫腓骨骨折术后小腿中上段骨及钢板外露、软组织缺损患者,缺损面积4 cm×8 cm～7 cm×12 cm,切取皮瓣面积6 cm×10 cm～9 cm×14 cm。结果 9例患者均获得随访,时间12～18个月。皮瓣均成活,皮瓣质地、颜色均良好。骨折愈合时间10～12个月。结论腓动脉穿支蒂顺行皮瓣血供可靠,层次清晰,操作简单,皮瓣转移幅度大,对小腿功能和外形影响小,是修复小腿中上段骨及钢板外露、软组织缺损创面的良好方法。

Objective To investigate the clinical curative effect of peroneal artery perforator pedicled with antegrade flap to repair upper calf bone and soft tissue defect of plate exposure. Methods The peroneal artery perforator pedi- cled flap was used for repair of 9 cases of patients with antegrade proximal tibial bone and soft tissue defect of plate exposure. The wound area were： 4 cm× 8 cm to 7 cm × 12 era, cutting flap area 6 cm × 10 cm - 9 cm ×14cm. Re- sults 9 patients were followed up for 12 - 18 months. All flaps survived, skin texture and color were good. Fracture healing time ranged from 10 to 12 months. Conclusions Sural artery periorator pedicled flap antegrade blood supply is reliable, the anatomy is clear, and is with simple operation, small influence on the function and shape, flap trans- fer of amplitude, repairing upper calf bone and soft tissue defect with plate exposure.
