撬拨复位植骨治疗跟骨骨折

Poking reduction and bone grafting for treatment of calcaneal fracture

目的探讨撬拨复位克氏针内固定小切口异体骨植骨治疗跟骨骨折的临床疗效。方法对18例波及距下关节面的跟骨骨折患者采用撬拨复位、小切口行异体骨植骨、克氏针固定治疗。结果患者伤口均一期愈合,无皮肤坏死和克氏针断裂发生。18例均获随访,时间3～12个月。术后Bhler角及Gissane角均较术前明显改善(P<0.05)。参照美国足踝外科学会足部功能评分系统评定:优12足,良5足,可1足。结论撬拨复位小切口植骨克氏针固定治疗跟骨关节内骨折疗效满意。

Objective To summarize the effect of poking reduction and Kirschner wire internal fixation with small in- cision bone allograft for the treatment of calcaneal fracture. Methods 18 patients with subtalar-articular caleaneal fracture were treated with poking reduction and small incision hone allograft and Kirschner wire fixation. Results All patients got primary healing without skin necrosis and Kirsehner wire fracture. 18 cases were followed up for 3 - 12 months. Postoperative B6hler angle and Gissane angle were significantly improved compared with preoperative （ P 〈 0. 05）. Assessed with reference to the American Orthopaedic Foot and Ankle Society foot function scoring system, the results were excellent in 12 feet, good in 5, and fair in 1. Conclusions Poking reduction and small incision bone allograft with＂ Kirschner wire fixation for treatment of intraarticular calcaneal fracture can provide satisfactory out- comes.
