23例原发性黏膜恶性黑色素瘤临床病理分析

A clinicopathologic study on 23 cases of primary mucosal melanoma

目的:探讨新疆地区原发性黏膜恶性黑色素瘤的临床病理学特征,诊断及鉴别诊断。方法:收集23例原发性黏膜恶性黑色素瘤,采用HE染色和免疫组化标记,进行临床病理分析。结果:原发性黏膜恶性黑色素瘤的组织结构及病理形态复杂多样。光镜下形态以多种细胞类型和组织形态混合存在为主要特征。细胞类型可分为:上皮样细胞型、梭形细胞型、小痣样细胞型、混合型。免疫组化标记瘤细胞表达S-100、KI-67、Vimentin均阳性表达,HMB-45、Melan-A分别为91%、86%。结论:原发性黏膜恶性黑色素瘤组织结构和病理形态表现复杂,部分病例诊断比较困难,需借助免疫组化诊断和鉴别诊断。

Objective：To explore the clinicopathologic features of primary mucosal melanoma of Xinjiang uygur.Methods：For 23 cases of primary mucosal melanoma,the clinicopathology analysis had been done by means of HE staining and immunohistochemistry.Results：The histopathological structures and morphological characters of primary mucosal melanoma were complex.The main features of melanoma under light microscopy were multiple cells type and histological features mix existence.The cell types include：epithelioid cell,spindle cell,nevus-like cell,mixed cell.The immunohistochemistry showed that for the melanoma the positive rate of HMB-45,S-100,KI-67,Vimentin,Melan-A were 91%,100%,100%,100% and 86% respectively.Conclusion：The clinical feature of melanoma are complex.Usually,it is difficult to diagnose.The diagnosis mainly depends on histopathological and immunohistochemical findings.
