围手术期抗生素应用的调查分析

Application and analysis of antibiotics in perioperation period

目的:调查分析围手术期抗生素的临床应用现状,为指导临床用药提供依据。方法:分析正阳县人民医院2010年10月至2011年10月收治的手术患者576例,对其围手术期抗生素的应用情况进行调查分析。分析手术类型、应用抗生素种类、时间及联合用药情况。结果:①手术类别为清洁手术39.58%(228/576),感染手术11.63%(67/576),污染手术48.79%(281/576)。②应用抗生素种类三代头孢类为33.16%(191/576),一、二代头孢类19.10%(110/576),喹诺酮类14.41%(83/576),氨基糖苷类13.37%(77/576),硝基咪唑类12.15%(70/576),大环内酯类3.99%(23/576),其它3.82%(22/576)。③应用抗生素时间:清洁手术(4.8±0.3)d,感染手术(8.1±0.2)d,污染手术(9.1±0.4)d。④抗生素:单用5.21%(30/576),二联55.73%(321/576),三联31.60%(182/576),四联7.46%(43/576)。结论:抗生素在围手术期的应用中存在不合理之处,应努力遵循指南并改进。

Objective： To investigate the status of antibiotics in perioperation period in order to guide the clinical medication. Methods ： The antibiotics of 576 cases who had surgery from October 2010 to October 2011 in perioperation period were analyzed according to the use of antibiotic types, time and combination. Results： ①Types of surgery as Clean procedures were 39. 58%（228/576）, Infection surgery 11.63 % （67/576）, Pollution surgery 48.79 % （ 281/576 ）. ②Use of antibiotic types： total three generations of eefazolin were 33.16% （191/576） , The first and second generation eephalosporin were 19.10% （ 110/576 ） , quinolone 14.41% （ 83/576 ） , aminoglycoside 13.37% （77/576） ,nitroimidazoles 12.15% （70/576） , Big ring laetone class 3.99% （23/ 576） ,others 3.82% （22/576）. ③time of using antibiotic ： Clean procedures（4.8±0.3） d, Infection surgery（8.1±0.2） d, Pollution surgery（9.1 ±0.4） d. ④antibiotics： single use 5.21% （30/576） , two combined 55.73% （321/576） , three combined 31.60% （182/576） , four combined 7.46% （43/576）. Conclusion： Application of antibiotics in perioperation period was improper and should be improved based on the guidelines.
