椭圆形撕囊在小切口非超声乳化白内障手术中的应用 被引量:2

摘要 目的探讨椭圆形撕囊在小切口非超声乳化白内障手术中的特点和安全性。方法对比椭圆形撕囊和传统正圆形撕囊在小切口非超声乳化手术中的效果和安全性。100例(100只眼)老年性白内障患者接受手术,随机分配至椭圆形撕囊组50只眼和正圆形撕囊组50只眼,记录术中、术后并发症,对比脱核时间,脱核次数以及术后3个月裸眼远视力。结果脱核时间:椭圆组(10.36±2.49)s,正圆组(15.06±2.46)s,差异具有统计学意义(t=~9.479,P=0.00);脱核次数(中位数):椭圆组1次,正圆组1.5次,差异具有统计学意义(Z=-3.513,P=0.00);术后3个月裸眼视力无统计学差异(t=-1.796,P=0.076)。结论椭圆形撕囊安全、有效,脱核效率高、时间短,也利于处理后囊膜破裂的术中并发症。 Objective To discuss the effect of oval capsulorhexis in small incision cataract surgery.Methods To compare clinical efficacy of oval capsulorhexis with conventional circular capsulorhexis in small incision cataract surgery.One hundred patients(100 eyes) with senile cataract were randomly assigned into two groups(50 eyes each group) : oval capsulorhexis group and conventional circular capsulorhexis group.Visual acuity,complications,average extracting nucleus time and number of attempts were observed.Results Extracting nucleus time: 10.36 ± 2.49 sec in oval capsulorhexis group and 15.06 ± 2.46 sec in conventional circular capsulorhexis group.There was significant difference between the two groups(t =-9.479,P = 0.00).Extracting nucleus attempts: average 1.0 in oval capsulorhexis group and 1.5 in conventional circular capsulorhexis group.There was also significant difference between the two groups(Z =-3.513,P = 0.00).There was no difference in visual acuity 3 months after operation between the two groups(t =-1.796,P = 0.076).Conclusion Oval capsulorhexis makes nucleus extraction more effective in small incision cataract surgery.This technique also makes it easier to manage with posterior capsule rupture.
