糖化血红蛋白与空腹血糖在诊断前驱糖尿病中的意义 被引量:36

摘要 目的比较糖化血红蛋白(HbA1C)(5.7%~6.4%)或空腹血糖受损(5.6~6.9 mmol/L)两项指标在诊断前驱糖尿病的患病率和评估在我国人群中用这两种指标诊断为前驱糖尿病患者发展为糖尿病的概率的差异。方法选进行健康体检并排除糖尿病(糖尿病诊断标准为空腹血糖≥7.0 mmol/L,自述有临床诊断的糖尿病,或HbA1C≥6.5%)的体检者为研究对象。检测根据空腹血糖受损(空腹血浆葡萄糖5.6~6.9 mmol/L)或HbA1C5.7%~6.4%,或两者兼有之诊断为前驱糖尿病,每年评估1次受检者发展为糖尿病的概率。结果 (1)2 800名体检者中,分别以HbA1C5.7%~6.4%、空腹血浆葡萄糖5.6~6.9 mmol/L及两者联合为诊断指标,前驱糖尿病患病率分别7%、20%、4%,分别以前两者诊断的前驱糖尿病患者的患病率比较,差异有统计学意义(P<0.05);(2)以上述不同指标诊断的前驱糖尿病患者5年内发展为糖尿病患者的概率分别为7%、9%、26%;以HbA1C5.7%~6.4%及空腹血浆葡萄糖5.6~6.9 mmol/L为诊断指标诊断的前驱糖尿病患者,5年内发展为糖尿病患者的概率,差异无统计学意义(P>0.05);使用两者联合诊断的前驱糖尿病患者,5年内发展为糖尿病的概率,与前两者比较,差异有统计学意义(P<0.05);(3)HbA1C达到6.1%后,5年内发展为糖尿病患者的概率明显升高。结论使用HbA1C5.7%~6.4%单独诊断前驱糖尿病,存在漏诊现象;6.1%可能是适合我国人群筛查糖尿病的HbA1C切点。
