hCG日血清雌二醇和孕酮升高对体外受精/卵胞浆内单精子注射妊娠结局的影响 被引量:3

摘要 目的探讨体外受精/卵胞浆内单精子注射(IVF/ICSI)周期控制性超排卵(COH)过程中注射人绒毛膜促性腺激素(hCG)日血清雌二醇(E2)和孕酮(P)水平对妊娠结局的影响。方法回顾性分析行长方案IVF/ICSI治疗的1 187个周期及后续473个冷冻移植(FET)周期的临床资料。按hCG日血清E2和P水平分为4组:E2<5 000 pg/mL且P<1.5 ng/mL(A组),E2<5 000 pg/mL且P≥1.5 ng/mL(B组),E2≥5 000 pg/mL且P<1.5ng/mL(C组),E2≥5 000 pg/mL且P≥1.5 ng/mL(D组),比较各组间各观察指标。结果各组间成熟卵率、受精率、卵裂率、优质胚胎率差异无统计学意义,A组和C组种植率(38.6%、31.0%vs 21.3%、22.0%,P<0.05)、临床妊娠率(52.3%、47.2%vs 29.5%、33.3%,P<0.05)显著高于B组和D组。C组和D组FET周期种植率、临床妊娠率显著高于A组和B组。结论 P和E2水平同时升高或仅P水平升高时种植率、妊娠率显著降低,不利于妊娠结局,仅E2水平升高对妊娠结局无影响。
