锁定加压钢板治疗C型人工髋关节置换术后假体周围骨折的疗效

Effects of locking compressive plate on type C periprosthetic femoral fracture following hip arthroplasty

目的：分析C型人工髋关节置换术后假体周围骨折的2种治疗方法的疗效。方法：对2007年8月至2011年8月广州医学院第三附属医院收治的15例Vancouver型C股骨假体周围骨折患者的治疗效果进行回顾性分析。患者分2组，1组行切开复位锁定加压钢板内固定术（切开复位组，7例）．另1组采用MIPPO技术经皮锁定加压铜板内固定（LISS组，8例）。结果：所有病例随访4～24个月，骨折均获愈合，对位对线艮好，平均愈合时间13．5周（12～20周），患膝Merchant评分，切开复位组优良率达57．1％：LISS组优良率达75．0％．结论：LISS治疗VancouverC型股骨假体周围骨折具有创伤小、出血量少、恢复好。

Objective： To compare the efficacy of 2 approaches for the treatment of type C periprosthetic femoral fracture following hip arthroplasty. Methods： Fifteen patients with Vancouver type C periprosthetic femoral fracture admitted to The Third Affiliated Hospital of Guangzhou Medical College between August 2007 and August 2008 were randomly assigned to be treated with locking compressive plate via open reduction （open reduction group, n = 7） or MIPPO technique （ group LISS, n = 8） for subsequent analysis on the recovery of bone fracture. Results： After a 4- to 24-month follow-up period, all patients had recovered from the fracture, as evidenced by satisfactory alignment and positioning of the fi＇actured lines. The average time to recovery was 13.5 （12-20） weeks. The Merchant scale of the knee joint rated as excellent and fair accounted for 57.1% in open reduction group and 75 % in LISS group. Conclusion ： Characterized by minimal injury and volume of hemorrhage and satisfactory recovery, LISS is a preferable approach for Vancouver type C periprosthetic femoral fracture.
