臭氧注射联合椎管外针刀闭合术治疗椎间盘源性下腰痛临床观察 被引量:3

A Clinical Study of Ozone Injection Combined with Extra-vertebra Needle Scalpel Close Loosening in Treatment of Discogenic Low Back Pain

摘要 目的:探讨中西医结合微创方法治疗椎间盘源性下腰痛的疗效。方法:我院2011年1月~2012年6月采用臭氧腰椎间盘盘内外注射或结合椎管外针刀闭合术治疗61例下腰痛患者。入选共72例诊断疑似为腰椎间盘源性下腰痛患者,随机分为2组:A组(臭氧注射)36例和B组(臭氧注射及椎管外针刀闭合术)36例,术后1周、6个月对两组患者疗效进行随访,根据视觉模拟评分(VAS)、改良Macnab法进行疗效评定。结果:A组29例、B组32例完成随访。两组治疗后1周、6个月VAS值较治疗前明显改变(P<0.01),术后1周VAS值2组比较差异无统计学意义(P>0.05);术后6个月2组VAS值比较(P<0.05);A组治疗后6个月优良率为65.5%,B组为78.1%,差异有统计学意义(P<0.05)。结论:臭氧注射治疗腰椎间盘源性下腰痛是安全可行的方法,结合针刀闭合术疗效满意。 Objective:To explore the curative effect of integrated traditional Chinese and western medicine and minimally in vasive surgery in treatment of discogenic low back pain. Methods:From January 2011 to June 2012, 61 patients with low back pain were treated by ozone injection combined with extra vertebra needle scalpel close loosening. 72 patients who were suspected discogenic lumbar pain were randomly divided into the A group (treated with ozone injection) and I3 group (treated with ozone injection combined with extra-vertebra needle scalpel close loosening), 36 cases in each group. At 1 week and 6months after operation, a curative effect assessment was performed according to visual analogue scales (VAS) and revised Macnab system respectively. Results:29 cases of group A and 32 patients of group B finished the follow up. VAS values before treatment were different significantly with that 1 week and 6 months after treatment in both groups. There was no sig nificant difference between the two groups in the VAS value one week after treatment. (P〉 0.05). The excellent rate was 65.5% in group A and 78.1% in group B 6 months after treatment. The difference is significant (P〈0.05). Conclusion:Ozone injection is a safe and effective method to treat the discogenic low hack pain. Especially combined with the extra-verte bra needle scalpel close loosening, it can improve the therapeutic effect.
