摘要
目的探讨上皮细胞黏附因子(EpCAM)在结直肠癌中的表达及其临床意义,探讨其表达与β-连环蛋白(β-catenin)表达的相关性。方法应用免疫组织化学法检测120例结直肠癌组织和30例正常结直肠组织中EpCAM的表达,同时在120例结直肠癌组织中检测β-catenin的表达,结合临床病理特征及随访资料进行统计学分析。结果EpCAM在结直肠癌组织中的高表达率约为60.8%,明显高于正常结直肠组织(6.7%,P〈0.01);EpCAM在结直肠癌组织中的表达与TMN分期、淋巴结转移、远处转移及复发密切相关(P〈0.05)。8.catenin在结直肠癌的高表达率约为70.0%,其表达与EpCAM的表达呈显著正相关(r=0.518,P〈0.01)。生存分析表明结直肠癌患者的预后与EpCAM的表达、淋巴结转移、远处转移及复发密切相关(P〈0.05);多因素分析表明EpCAM、TNM分期及淋巴结转移可作为结直肠癌患者的独立预后因素。结论EpCAM在结直肠癌中的表达与β-catenin的表达呈明显正相关。
Objective To investigate the expression and clinical significance of epithelial cell ad- hesion molecule (EpCAM) , and its correlation with β-catenin in colorectal cancer. Methods Immunohis- tochemical method was used to detect the EpCAM expression in 120 cases of colorectal cancer and 30 cases of normal colorectal tissues, and meanwhile detect the 13-catenin expression in 120 cases of colorectal canc- er tissues. The results combined with clinicopathological features and follow-up data were analyzed statisti- cally. Results The rate of high EpCAM expression in colorectal carcinoma tissue (60. 8% ) was obviously higher than that in normal colorectal tissue ( 6. 7% , P 〈 0. 01 ). The expression of EpCAM in colorectal carcinoma tissue was associated with TMN stages, lymph node metastasis, distant metastasis and recurrence ( P 〈 0. 05 ). The rate of high β-catenin expression in colorectal carcinoma tissue was 70%, and signifi- cantly correlated with the expression of EpCAM (r = 0. 518, P 〈 0. 01 ). Survival analysis showed that the prognosis of patients with colorectal cancer was associated with EpCAM expression, lymph node metastasis, distant metastasis and recurrence (P 〈 0. 05 ). Multiple factor analys.is showed that EpCAM, TNM stages and lymph node metastasis could be used as independent prognostic factors for colorcctal cancer patients. Conclusion The EpCAM expression has a significantly positive correlation with the β-catenin expression in colorectal cancer.
