无创通气应用于呼吸衰竭患者支气管镜检查的研究

Application of noninvasive ventilation in bronchoscopy of patients with respiratory failure

目的评价无创通气对呼吸衰竭患者支气管镜检查的安全性。方法对比观察22例呼吸衰竭患者行支气管镜检查术中分别应用高频通气给氧与无创通气吸氧后的心率、血氧饱和度、二氧化碳分压的变化情况。结果高频通气组的患者在术中的心率、血二氧化碳分压、血氧饱和度较术前有明显变动,差异显著(P均<0.05)。患者接受无创通气给氧后心率、血氧饱和度与术前水平变化不明显(P>0.05),血二氧化碳分压无明显升高。结论呼吸衰竭患者支气管镜检查时应用无创通气组较高频通气吸氧组患者术中生命体征更平稳,风险更小。

Objective To evaluate the safety of noninvasive ventilation on bronchoscopy of patients with respiratory failure. Methods All 22 cases of respiratory failure patients in the two groups respectively received high frequency ventilation to oxygen or noninvasive ventilation oxygen in the hronchoscopy, then contrasted the heart rate, blood oxy- gen saturation and the change of CO2 partial pressure. Results The heart rate, blood carbon dioxide partial pressure and blood oxygen saturation in the high frequency ventilation group were obviously changed between pre-operation and during operation （P all 〈 0.05）, and in the noninvasive ventilation oxygen group were not obviously changed between pre-operation and during operation （P 〉 0.05）. Conclusion The vital signs of patients with respiratory failure in nonin- vasive ventilation oxygen group are more stable and less risky than the high frequency ventilation group in bronchoscopy.
