低浓度钆对比剂在双下肢磁共振动脉成像中的应用研究

摘要 [目的]探讨低浓度钆对比剂在双下肢磁共振动脉成像中的应用价值。[方法]选取临床怀疑下肢动脉狭窄行磁共振双下肢动脉成像的患者110例,根据对比剂的浓度随机分为低浓度组和常规浓度组。主、客观评价两组患者腹主动脉下段及下肢各级动脉显示情况及图像质量。[结果]与常规组比较,低浓度组在各段血管的显示率及图像质量均能满足诊断要求,并且小腿静脉污染程度较轻(P>0.05)。与常规组比较,低浓度组在腹主动脉、髂动脉、股动脉及腘动脉的客观评价即信号强度(signal intensity,SI)、信噪比(signal-to-noise ratio,SNR)及对比噪声比(contrast-to-noiseratio,CNR)无明显差异(P>0.05),但在腓动脉及胫前、胫后动脉的客观评价上存在统计学差异(P<0.05)。[结论]低浓度钆对比剂在双下肢磁共振动脉成像中图像质量能满足诊断要求,具有一定的应用价值。 [Objective] To evaluate the clinical value of contrast enhanced magnetic resonance angiography(CE MRA) by using low-concentration gadopentetate dimeglumine injection in diagnosing abnormalities of the lower extremities.[Methods] 110 patients who were suspected arterial abnormalities of the lower extremities were randomly divided into two groups: one group with low-concentratio(n=58),another with routine-concentration(n=52).The image quality of artery from the lower segment of abdomen to the lower limbs in the two groups was evaluated subjectively and objectively.[Results] Compared with the routine-concentration group,the displayed rate of segmented vessel and the image quality in the low-concentration group could reach diagnostic requirements(P&gt;0.05),and reduced the effect of contamination from veins of lower extremities(P&gt;0.05).There was no significant difference(P&gt;0.05) in the objective scores(SI,SNR and CNR) of the abdominal aorta,iliac artery,femoral artery and the popliteal artery between the low-con centration and the routine-concentration group.The objective score of the peroneal artery,anterior and the posterior tibial artery existed significant differ ences(P&lt;0.05).[Conclusion] The imaging quality of MR angiography by using low concentration gadopentetate dimeglumine injection can be used in clin ical practice of arterial disorders of lower extremities.
