右美托咪定对直肠癌患者术中循环系统、凝血机能和失血的影响

Effect of dexmedetomidine on circulatory system,blood clotting function and blood loss in patients with rectal cancer

目的：探讨右美托咪定对直肠癌手术患者术中失血和凝血的影响。方法：40例直肠癌患者随机分为右美托咪定（D）组和对照（N）组,每组20例。D组静注右美托咪定0.5μg/kg后以0.2~0.5μg.kg-1.h-1的速度输注,N组使用生理盐水。分别记录用药前（T0）、用药后5min（T1）、10min（T2）、15min（T3）、30min（T4）心率、平均动脉压;术中失血量;用药前后凝血和血小板指标变化。结果：右美托咪定组用药后15min心率下降最明显（P〈0.05）,术中失血量明显高于对照组（P〈0.05）。结论：右美托咪定能明显降低心率,对血压及凝血功能的影响小,但可能增加直肠癌患者术中失血量。

Objective： To explore the effect of dexmedetomidine（DEX） on intraoperative blood loss and clotting in patients with rectal cancer.Methods： 40 patients with rectal cancer were randomly divided into DEX（D） group and the control（N）group,n= 20.In D group,DEX 0.5μg/kg was injected intravenously and followed by continuous infusion at 0.2~0.5μg.kg-1.h-1.In the control,NS was used instead.Heart rate,mean arterial pressure,total blood loss,clotting tests,and platelet count were assessed and recorded.Results： The heart rate decreased significantly 15 min after administration of DEX in group D（P0.05）.The intraoperative blood loss was significantly higher in group D than that in group N（P0.05）.Conclusion： DEX has little effect on blood clotting function,but may increase blood loss in patients with rectal cancer.
