采用棘突及椎板进行椎间植骨融合内固定治疗腰椎疾病

Treatment of Lumbar Disease with Internal Fixation of Intervertebral Bone Grafting Fusion Utilizing Spinous Process and Vertebral Laminae

目的:评价采用棘突及椎板进行椎间植骨融合内固定治疗腰椎疾病的临床疗效。方法:回顾性分析自2009年2月～2012年2月期间,我院采用棘突及椎板做为植骨源进行椎间植骨融合内固定治疗226例腰椎疾病患者的临床资料,根据术前、术后、随访时的临床症状、体征及脊柱标准侧位X线片,观察植骨融合及椎间塌陷的情况。结果:经过3～18个月(平均大约10.5个月)随访,椎间隙高度令人满意,植骨融合时间平均在5.5个月,脊柱融合率为97.5%,平均手术时间缩短20 min。结论:采用棘突及椎板做为植骨源进行椎间植骨融合内固定治疗腰椎疾病,能提高植骨融合率,避免自体取骨带来的并发症及利用骨替代物的高额费用,经济简便,缩短手术时间,减少手术带来的并发症,是一种安全、可行、有效的好办法。

Objective：To evaluate the clinical effects of treating lumbar vertebrae diseases with internal fixation of intervertebral bone grafting fusion utilizing spinous process and vertebral laminae.Methods：Between February,2009 and February,2012,our hospital made the retrospective analysis of 226 cases of lumbar vertebrae diseases treated with internal fixation of intervertebral bone grafting fusion using spinous process and vertebral laminae as bone graft source.Based on the symptoms,signs and radiographs of standard lateral spine before and after operation and during the period of follow-up,we observed the conditions of bone grafting fusion and intervertebral collapse.Results：The height of intervertebral space was satisfactory after 3 months to 18 months（approximately 10.5 months on the average）of follow-up.Bone graft fusion time was 5.5 month.The rate of spinal fusion was 97.5%.The average decrease in the operation time was 20 minutes.Conclusions： There was a higher fusion rate of bone graft among the patients with lumbar vertebrae diseases treated with internal fixation of intervertebral bone grafting fusion using spinous process and vertebral laminae as bone graft source.It could avoid complications caused by autogenous graft and the high cost of using bone substitute.It is a safe,feasible and effective method which can decrease the operation time,reduce the complications of operation and be expected to be economical and simple.
