早期全静脉营养对低体质量新生儿营养状态及生化指标的影响研究

Influence of Early Total Parenteral Nutrition on the Nutrition State and Biochemical Indexes of Low-birth-weight Infants

目的探讨早期全静脉营养对低体质量新生儿营养状态及生化指标的影响。方法选取2009年5月—2012年9月本院行早期全静脉营养干预的低体质量新生儿32例为观察组,同期行传统静脉营养干预的低体质量新生儿32例为对照组,对比两组新生儿干预前及干预后7 d的生长发育、营养状态及生化指标。结果干预前两组新生儿体质量、身长、上臂围及血清前清蛋白、胆红素、尿素氮、肌酐、红细胞C3b受体花环率(RBC-C3bRR)、RBC-C3b花环抑制率(RFIR)、RBC-C3bR花环促进率(RFER)、锌、铜、铁、钙、镁水平比较,差异均无统计学意义(P>0.05);干预后7 d观察组较对照组体质量、身长、上臂围增加,血清前清蛋白、RBC-C3bRR、RFER、锌、铜、铁、钙、镁水平升高,胆红素、尿素氮、肌酐、RFIR水平降低,差异均有统计学意义(P<0.05)。结论早期全静脉营养可改善低体质量新生儿的营养状态及生化指标,可有效改善低体质量新生儿的综合状态。

Objective To study the influence of early total parenteral nutrition on the nutrition state and biochemicalindexes of low - birth - weight infants. Methods 32 low - birth - weight infants who were treated with early total parenteral nutrition in our hospital from May 2009 to September 2012 were selected as observation group,and 32 low - birth - weight infants whowere treated with traditional parenteral nutrition at the same time were selected as control group. Then the growth,nutrition stateand biochemical indexes of two groups before intervention and 7 days after intervention were compared. Results Before intervention, the body weight, body length, upper - arm circumference, prealbumin, bilirubin, urea nitrogen, creatinine, RBC -C3bRR, RFIR, RFER, zinc, copper, ferrum, calcium and magnesium between the two groups showed no statistically significant difference （ P 〉 0. 05 ） . The body weight, body length, upper - arm circumference of observation group 7 days after intervention were all higher than those of control group, and the prealbumin, RBC - C3bRR, RFER, zinc, copper, ferrum, calcium andmagnesium were also higher than those of control group. The bilirubin,urea nitrogen,creatinine,and RFIR were all lower thanthose of control group （ all P 〈 0. 05 ） . Conclusion The early total parenteral nutrition can effectively improve the nutrition stateand biochemical indexes of low - birth - weight infants and it can also effectively improve the comprehensive sate of low - birth -weight infants.
