穴位埋入药线联合李可“乌蛇荣皮汤”治疗寻常型银屑病的临床疗效及其对CD_4^+和CD_8^+ T细胞的影响

Curative Effect of Acupoint-embedding Therapy with Catgut of Traditional Chinese Medicine Combined with LI Ke Wushe Rongpitang on Psoriasis Vulgaris and Its Influence on the Transformation of CD_4^+ and CD_8^+ T-lymphocyte

目的观察穴位埋入药线联合李可"乌蛇荣皮汤"治疗寻常型银屑病的临床疗效,探讨其对CD4+和CD8+T细胞的影响。方法选择2004年9月—2011年12月本院收治的寻常型银屑病(风热血燥型白疕)患者共106例,采用分层随机分组方法分为对照组30例和试验组76例,同时选取与试验组患者相匹配的健康人30例为健康对照组。对照组采用复方氨肽素片进行治疗,试验组采用穴位埋入药线联合李可"乌蛇荣皮汤"进行治疗;两组疗程均为8周。观察两组患者严重程度指数(PASI)变化并评价临床疗效,检测试验组患者治疗前后CD4+和CD8+T细胞水平并与健康对照组进行比较。结果治疗前对照组PASI为(17.79±7.36),试验组为(18.46±7.28),差异无统计学意义(t=0.43,P>0.05);治疗后对照组PASI为(7.46±2.63),试验组为(4.82±2.78),差异有统计学意义(t=4.47,P<0.001)。试验组总有效率(88.2%,67/76)高于对照组(63.3%,19/30),差异有统计学意义(χ2=8.66,P<0.01)。试验组治疗前CD4+和CD8+T细胞水平与健康对照组比较,差异均有统计学意义(P<0.01);试验组治疗后CD4+T细胞水平与健康对照组比较,差异无统计学意义(P>0.05),CD8+T细胞水平与健康对照组比较,差异有统计学意义(P<0.01)。结论穴位埋入药线联合李可"乌蛇荣皮汤"治疗寻常型银屑病具有较好的临床疗效,可提高患者CD4+T细胞水平,改善患者免疫功能。

Objective To investigate the curative effect of acupoint - embedding therapy with catgut of TCM combined with O Ke Wushe rongpitang and its influence on CD4 ＋ and CD8 ＋ T - lymphocyte in the treatment of psoriasis vulgaris. Methods106 cases of psoriasis （wind - heat and blood dryness type） admitted to our hospital from September 2004 to December 2011were randomly divided into control group （30 cases） , treated only with Di Yin Pian, and experiment group （76 cases） , treated with acupoint - embedding therapy with catgut of TCM and LI Ke Wushe rongpitang. After 8 weeks of continuous treatments,the clinical curative effects were evaluated according to the severity index （ PASI） . The CD4 ＋ and CD8 ＋ T - lymphocyte levelswere determined before and after treatment in the experiment group. A comparison was made between the two groups and with thatof the group of thirty healthy people. Results The PASI before treatment in control group and experimental group was （17. 79±7. 36） and （18. 46 ±7. 28） , respectively, and the difference was not statistically significant （t = 0.43, P 〉 0. 05） . Aftertreatment, the figure was （7. 46 ±2.63） and （4. 82 ±2.78） respectively, and the difference was statistically significant （t =4. 47,P 〈0. 001 ） . The curative effect of the experiment group and control group, （88. 2%,67/76） and （63. 3%,19/30）respectively, showed statistically significant difference （χ^2 = 8. 66, P 〈 0. 01 ） . Before treatment, the CD4 ＋ and CD8 ＋ T -lymphocyte in experiment group showed statistically significant difference compared with the healthy control group （P 〈 0. 01 ）.However, after treatment, the CD4 ＋ T - lymphocyte in experiment group showed no statistically significant difference comparedwith the healthy control group （P 〉0. 05）,but CD8 ＋ T - lymphocyte showed （P 〈0. 01） . Conclusion It is effective to treatpsoriasis vulgaris and improve the immunoregulatory function with acupoint - embedding therapy with catgut of TCM combined with LI Ke Wushe rongpitang.
