椎弓根钉内固定系统结合椎间融合治疗严重腰椎滑脱的临床效果

The Clinical Effect of Pedicle Screw Fixation Combined with Interbody Fusion in Treatment of Severe Lumbar Spondylolisthesis

目的:观察椎弓根钉内固定系统结合椎间融合治疗严重腰椎滑脱的临床疗效。方法:从笔者所在医院收治入院的严重腰椎滑脱患者中抽取90例作为研究对象,随机分为A、B、C三组,A组使用椎弓根钉内固定系统与椎间融合相结合治疗,B组单独使用椎弓根钉内固定治疗,C组单独使用椎间融合器治疗。对比观察三组患者的临床治疗效果。结果:三组患者治疗后各项指标均明显改善,A组JOA评分优良率明显高于B、C两组,随访中B、C两组的复位率、椎间隙与椎间孔高度明显低于A组,滑脱率明显高于A组,差异均有统计学意义(P<0.05)。结论:对严重腰椎滑脱患者使用椎弓根钉内固定系统与椎间融合进行治疗的临床疗效肯定,两种方法相结合比单独应用具有更为理想的治疗效果,值得进一步推广应用。

Objective： To observe the effect of pedicle screw internal fixation system combined with interbody fusion in treating severe lumbar spondylolisthesis intervertebral fusion.Methods： 90 cases as study object were selected from patients with severe lumbar spondylolisthesis which were treated in our hospital from admission.90 cases were randomly divided into A, B, C three groups, A group treated with pedicle screw system internal fixation and interbody fusion in the combination therapy group, B group treated with pedicle screw fixation, C group alone treated with the interbody fusion cage.The efficacy of the theree groups of patients were compared.Results： The all indexes were significantly improved after treatment in three groups, JOA score was excellent and good rate was significantly higher in A group than that of B, C two groups, the rate of reset, slip rate, intervertebral height and foraminal height in B and C groups were higher than that in A group, the differences were statistically significant（P〈0.05）.Conclusion： The clinical curative effect of severe lumbar spondylolisthesis with pediele screw fixation and interbody fusion for the treatment system, two kinds of methods with combination therapy than used alone, with better therapeutic effect, is worthy of further popularization and application.
