HBV相关慢加急性肝功能衰竭患者血清IL-4和IL-10的表达及临床意义

Expression and significance of interleukin-4 and interleukin-10 in patients with HBV-related acute-on-chronic liver failure

目的探讨乙型肝炎病毒相关慢加急性肝功能衰竭(HBV-ACLF)患者体内抗炎细胞因子白细胞介素(IL)-4和IL-10的表达及其临床意义。方法收集兰州市第二人民医院2010年10月至2012年3月收治的HBV-ACLF患者共33例(HBV-ACLF组),同时选取同期住院的慢性乙型肝炎(CHB)患者42例(CHB组)和20例健康体检者(健康对照组)入组。采用ELISA法检测3组人群血清中IL-4和IL-10水平,同时检测各项临床指标。结果 HBV-ACLF组患者IL-4和IL-10水平均低于健康对照组,IL-10水平低于CHB组(P均<0.05),治疗4周后复查IL-4和IL-10水平显著上升,较治疗前差异具有统计学意义(P均<0.05);CHB组患者IL-4和IL-10水平低于健康对照组,治疗前后IL-4和IL-10水平差异无统计学意义(P均>0.05)。结论 HBV-ACLF患者IL-4和IL-10表达水平显著低下,监测其动态变化对预测HBV-ACLF患者的疾病进展和预后有重要的临床意义。

Objective To investigate the expression and clinical significance ofinterleukin （IL）-4 and IL-10 levels in patients with HBV-related acute-on-chronic liver failure （HBV-ACLF）. Methods Total of 33 patients with HBV-ACLF （HBV-ACLF group） and 42 patients with chronic hepatitis B （CHB） （CHB group）, who were hospitalized in the Sencond People＇s Hospital of Lanzhou from October 2010 to March 2012 were enrolled. While 20 healthy people （HC group） were taken as controls in the study. ELISA was applied to detect the leveis of IL-4 and IL-10 in the 3 groups. Meanwhile, clinical parameters were investigated, respectively. Results The levels of IL-4 and IL-10 in HBV-ACLF group were significantly lower than those in CHB group and HC group （P 〈 0.05）, and the level of IL-10 in HBV-ACLF group was significantly lower than that in CHB group （P 〈 0.05）. In HBV-ACLF group, the levels of IL-4 and IL-10 increased before and after treatment for 4 weeks （P 〈 0.05）. The levels of IL-4 and IL-10 in CHB group were significantly lower than those in HC group （P 〈 0.05）. There were no significant change of IL-4 and IL-10 levels before and after treatment for 4 weeks in CHB group （P 〉 0.05）. Conclusions The serum levels of IL-4 and IL-10 were significantly lower in HBV- ACLF patients. Monitoring of the dynamic change of IL-4 and IL-10 have a good clinical significance in disease progression and prognosis of HBV-ACLF patients.
